In an era of glocalization, how might educational systems cultivate classroom connections embracing diverse student gifts? This arts-based narrative inquiry explores fatherly life lessons of 17 undergraduate and six graduate students enrolled in an introductory qualitative research course at a large urban Chinese university. Building on the course instructor's model, students engaged in arts-based narrative inquiry to develop children's books on treasured fatherly life lessons that they then shared with second grade students at a local Chinese school. Drawing upon the "Confucian Analects" and Laozi's "Tao Te Ching," this study evidences empathy as rooted across cultures and ecologies, and that many fatherly life lessons take place in natural settings. This study encourages teacher education practice and research to engage arts-based autobiographical inquiry, and to explore empathy conceptualizations and expressions across cultures and ecologies. As "glocalization" brings together diverse groups, this work is important to create shared spaces for international connection and meaningful inter-institutional explorations.Item Association Between Empathy and Burnout Among Emergency Medicine Physicians(Elmer Press, 2019-07) Wolfshohl, Jon A.; Bradley, Keegan; Bell, Charles; Bell, Sarah; Hodges, Caleb; Knowles, Heidi; Chaudhari, Bharti R.; Kirby, Ryan; Kline, Jeffrey A.; Wang, Hao; Emergency Medicine, School of MedicineBackground: The association between physician self-reported empathy and burnout has been studied in the past with diverse findings. We aimed to determine the association between empathy and burnout among United States emergency medicine (EM) physicians using a novel combination of tools for validation. Methods: This was a prospective single-center observational study. Data were collected from EM physicians. From December 1, 2018 to January 31, 2019, we used the Jefferson scale of empathy (JSE) to assess physician empathy and the Copenhagen burnout inventory (CBI) to assess burnout. We divided EM physicians into different groups (residents in each year of training, junior/senior attendings). Empathy, burnout scores and their association were analyzed and compared among these groups. Results: A total of 33 attending physicians and 35 EM residents participated in this study. Median self-reported empathy scores were 113 (interquartile range (IQR): 105 - 117) in post-graduate year (PGY)-1, 112 (90 - 115) in PGY-2, 106 (93 - 118) in PGY-3 EM residents, 112 (105 - 116) in junior and 114 (101 - 125) in senior attending physicians. Overall burnout scores were 43 (33 - 50) in PGY-1, 51 (29 - 56) in PGY-2, 43 (42 - 53) in PGY-3 EM residents, 33 (24 - 47) in junior attending and 25 (22 - 53) in senior attending physicians separately. The Spearman correlation (ρ) was -0.11 and β-weight was -0.23 between empathy and patient-related burnout scores. Conclusion: Self-reported empathy declines over the course of EM residency training and improves after graduation. Overall high burnout occurs among EM residents and improves after graduation. Our analysis showed a weak negative correlation between self-reported empathy and patient-related burnout among EM physicians.Item Brick a Brick: Using Applied Improvisation to Build Empathy in Design Facilitators(2019) Meschi, Maria; Napier, PamelaThe surge in design thinking and people-centered design worldwide has given rise to a new role for designers: design facilitator. A design facilitator is leading diverse groups of participants through the design process, providing opportunities for them to share their perspectives, and guiding them as they contribute to design solutions. These engagements require highly developed interpersonal skills, but few interventions exist to aid designers in cultivating these skills. Other disciplines have turned to applied improvisation, an approach derived from improvisational theatre, to improve communication, collaboration and other dimensions of social-emotional learning. This research examines how an applied improvisation approach might be utilized to cultivate empathy in design facilitators. To answer this question, participatory action research was conducted with design facilitators, applied improvisation facilitators, and an instructional designer. Interviews with facilitators from both contexts resulted in a model of empathy, evidence in support of the applied improvisation approach, and perceived barriers to implementation of applied improvisation in the design context. A participatory design session with facilitators from both contexts explored the specific actions that contribute to empathic facilitation and generated conceptual prototypes of an empathic facilitation training program. Finally, primary and secondary research were synthesized to create a solution prototype that was evaluated by an instructional designer and submitted to design conferences for peer review. The outcome of this research is a conceptual framework for a training program entitled, Improv for Empathic Facilitation. The solution is founded upon an experiential learning model and scaffolds learners through developing skills in four competencies: self-awareness, social awareness, collaboration, and facilitating with empathy. In addition to applied improvisation-based training, learners engage in simulated facilitation scenarios in order to practice their skills. Finally, the program utilizes a series of formative assessments by engaging in critical reflection throughout and culminates in a summative assessment at the conclusion. Criteria for the assessments is learner-generated throughout the program, honoring both their experience and expertise. This research provides a model of how to explore the cultivation of interpersonal skills in design facilitators. Additionally, by presenting its potential impact on interpersonal skills rather than cognitive skills, this research highlights a new dimension of how the fields of design and improvisation might positively impact each other.Item Can Text Messages Increase Empathy and Prosocial Behavior? Can we use mobile technology to make people more empathic? It may depend on how empathy is measured. Scholars have identified a number of different facets and correlates of empathy. This study is among the first to take a comprehensive, multidimensional approach to empathy to determine how empathy training could affect these different facets and correlates. In doing so, we can learn more about empathy and its multifaceted nature. Participants (N = 90) were randomly assigned to receive either an empathy-building text message program ( Text to Connect ) or one of two control conditions (active versus passive). Respondents completed measures of dispositional empathy (i.e. self-perceptions of being an empathic person), affective empathy (i.e. motivations to help, immediate feelings of empathic concern), and prosocial behavior (i.e. self-reports and observer-reports) at baseline, and then again after the 14 day intervention period. We found that empathy-building messages increased affective indicators of empathy and prosocial behaviors, but actually decreased self-perceptions of empathy, relative to control messages. Although the brief text messaging intervention did not consistently impact empathy-related personality traits, it holds promise for the use of mobile technology for changing empathic motivations and behaviors.Item Can text messages make people kinder?(Oxford UP, 2014) Konrath, Sara H.Empathic character is a set of interrelated dispositions, skills, motivations, emotions, and behaviors that involve a habitual responsiveness to others’ needs. It is linked to higher prosocial behavior, lower aggression, and better health. There is much research demonstrating both its consistency within people over time and its malleability in response to environmental and situational cues, including face-to-face interventions. In this paper I examine whether it is possible to increase empathic character using a mobile-based program. A large body of research in public health has used text messages to improve physical health outcomes and behaviors, but no research has examined whether text messages can be used to change traits. We conducted a study (N=90) in which participants received 6 daily empathy-building text messages for a 2 week period, versus a control group. We found that those in the empathy group showed some evidence of increased empathic character compared to control participants. The chapter ends by discussing implications of this work for a deeper understanding of empathic character, and some future directions of this research.Item Critical Synthesis Package: Interpersonal Reactivity Index (IRI)(2013) Konrath, Sara H.The IRI measures four different dimensions of dispositional empathy: 1) The Empathic Concern subscale assesses emotional empathy, or feelings of compassion for others in distress (e.g. “I often have tender, concerned feelings for people less fortunate than me.”) 2) The Perspective Taking subscale assesses cognitive empathy, or the tendency to see the world from others’ viewpoints (e.g. “I sometimes try to understand my friends better by imagining how things look from their perspective.”) 3) The Personal Distress subscale assesses self-focused responses to others’ suffering (e.g. “When I see someone who badly needs help in an emergency, I go to pieces.”) 4) The Fantasy subscale assesses empathy for fictional characters (e.g. “I really get involved with the feelings of the characters in a novel.”)Item Development and validation of the Single Item Trait Empathy Scale (SITES)(Elsevier, 2018-04) Konrath, Sara; Meier, Brian P.; Bushman, Brad J.; Lilly Family School of PhilanthropyEmpathy involves feeling compassion for others and imagining how they feel. In this article, we develop and validate the Single Item Trait Empathy Scale (SITES), which contains only one item that takes seconds to complete. In seven studies (N=5,724), the SITES was found to be both reliable and valid. It correlated in expected ways with a wide variety of intrapersonal outcomes. For example, it is negatively correlated with narcissism, depression, anxiety, and alexithymia. In contrast, it is positively correlated with other measures of empathy, self-esteem, subjective well-being, and agreeableness. The SITES also correlates with a wide variety of interpersonal outcomes, especially compassion for others and helping others. The SITES is recommended in situations when time or question quantity is constrained.Item Development of an empathy and clarity rating scale to measure the effect of medical improv on end-of-first-year OCSE performance: a pilot study(Taylor & Francis Open, 2019-12) Terregino, Carol A.; Copeland, H. How do you teach and measure empathy? Medical educators, including those inspired by Alan Alda, have turned to theater to teach skills in empathetic communication. Improvisation-informedcurriculum (medical improv) draws upon foundational actors training: deep listening, emotional understanding, connections, authenticity. Arating scale to measure the impact of medical improv on empathetic and clear communication does not exist. Objective: To develop aframework and instrument, the Empathy and Clarity Rating Scale (ECRS), for measuring communication elements used by actors and physicians, and pilot ECRS to test effectiveness of medical improv on first-yearstudents' communication skills. Design: Four medical schools collaborated. USMLE Step 2 Communication and Interpersonal Skills (CIS) domains were used as framework for discussion among three focus groups, each with clinicians, actors, communication experts, and community members with patient experience. Audiotaped discussions were transcribed; open coding procedures located emerging themes. The initial coding scheme was compared with the Consultation and Relational Empathy (CARE) measure. ECRS content was aligned with CARE, CIS and focus group themes. Modified nominal processes were conducted to finalize the scale. We implemented procedures to establish content validity and interrater reliability. Final ECRS was used to study student performance across three levels of experience with medical improv. Results: The final ECRS was comprised of seven five-pointscale items. Narrative comments precede behaviorally anchored ratings: 5=desired, 1=ineffective, 2-4=developing based upon adjustment needed. Rater agreement across all items was 84%. There was asmall correlation between the ECRS and another measure interviewing (r=0.262, p=0.003). Students with advanced medical improv training outperformed those without (F=3.51, p=.042). Conclusion: Acommunication scale enlightened by experiences of actors, clinicians, scholars and patients has been developed. The ECRS has potential to detect the impact of medical improv on development of empathetic and clear communication.Item Empathy and stress related neural responses in maternal decision making(Frontiers, 2014-06-12) Ho, S. Shaun; Konrath, Sara H.; Brown, Stephanie; Swain, James E.Mothers need to make caregiving decisions to meet the needs of children, which may or may not result in positive child feedback. Variations in caregivers' emotional reactivity to unpleasant child-feedback may be partially explained by their dispositional empathy levels. Furthermore, empathic response to the child's unpleasant feedback likely helps mothers to regulate their own stress. We investigated the relationship between maternal dispositional empathy, stress reactivity, and neural correlates of child feedback to caregiving decisions. In Part 1 of the study, 33 female participants were recruited to undergo a lab-based mild stressor, the Social Evaluation Test (SET), and then in Part 2 of the study, a subset of the participants, 14 mothers, performed a Parenting Decision Making Task (PDMT) in an fMRI setting. Four dimensions of dispositional empathy based on the Interpersonal Reactivity Index were measured in all participants-Personal Distress, Empathic Concern, Perspective Taking, and Fantasy. Overall, we found that the Personal Distress and Perspective Taking were associated with greater and lesser cortisol reactivity, respectively. The four types of empathy were distinctly associated with the negative (vs. positive) child feedback activation in the brain. Personal Distress was associated with amygdala and hypothalamus activation, Empathic Concern with the left ventral striatum, ventrolateral prefrontal cortex (VLPFC), and supplemental motor area (SMA) activation, and Fantasy with the septal area, right SMA and VLPFC activation. Interestingly, hypothalamus-septal coupling during the negative feedback condition was associated with less PDMT-related cortisol reactivity. The roles of distinct forms of dispositional empathy in neural and stress responses are discussed.
