    Bond strength evaluation of two resin cements with two adhesives and analysis of mode of failure

    Cementing of indirect restorations with resin cements generally requires the pre-treatment of dentin with an adhesive. When dual-cured or chemical-cured resin cements are used with these single-step adhesives, incompatibility issues exist. This has resulted in manufacturers making chemical changes in their products. Kerr Dental markets a new resin cement, Nexus Third generation (NX3), which utilizes a proprietary redox system different from the second generation of composite luting agent (NX2). The aim of this study was to evaluate microtensile bond strength and mode of failure of NX3 and NX2 with two different adhesive systems (total-etch and self-etch) after 1 week and after 3 months of storage. Methods: Sixty-four non-carious teeth were sectioned to expose the dentin using a low-speed saw. Dentin surfaces were ground with 320-grit SiC paper. The adhesives Optibond Solo Plus (SOL), and Optibond All In One (AIO) were applied, and resin cements (NX2, NX3) were used to lute 4-mm composite discs to the treated dentin surfaces. Microtensile bond strength was determined at 1 week (IM) and after 3 months (3MON) of storage using a universal testing machine (MTS). All specimens were examined under the stereomicroscope to determine the mode of failure. Random specimens from each failure group were examined using scanning electron microscopy. Statistical Analysis: Comparisons between the treatment combinations for differences in microtensile bond strength were performed using Weibull-distribution survival analysis. Comparisons between the treatment combinations for differences in the failure mode were performed using Fisher’s Exact tests. The group NX3 SOL IM (30.5 MPa) had significantly higher bond strength than NX3 SOL 3MON (13.4 MPa); NX3 AIO IM (11.3MPa); NX3 AIO 3MON (8.2 MPa; NX2 AIO 3MON (5.8 MPa); NX2 SOL IM (6.3 MPa), and NX2 SOL 3MON (3.2 MPa). The group NX2 AIO IM (19.3 MPa) was not significantly different from NX3 SOL IM. The group NX2 SOL 3MON and group NX2 SOL IM had a significantly higher percentage of teeth with mixed failure than all of the other groups. None of the other groups had significantly different failure mode. The group NX3 SOL IM had 90-percent beam survival beyond 17 MPa, and NX2 AIO IM had 50 percent of beams surviving beyond 17 MPa, a better performance. For all the other groups, more than 50 percent of beams failed below 17 MPa. Results show high evidence of degradation for all groups considered in this investigation. The use of these types of cement adhesive combinations in clinical situations should be used with this understanding.

    Clinical Evaluation of a Universal Adhesive in Non-Carious Cervical Lesions

    The “total-etch” or “etch-and-rinse” systems have been the gold standard of dental bonding for decades. However, these systems are very technique-sensitive and time-consuming compared to newer “self-etch” or “self-adhesive” systems and have been implicated in cases of postoperative sensitivity. The purpose of this study was to compare the effects of two surface treatment protocols (self-etch vs. selective-etch) on the clinical performance of a universal adhesive and resin composite in Class V non-carious cervical lesions (NCCLs). Thirty-three volunteer subjects (17 male; 16 female; age range = 20 to 75 years) having at least two NCCLs were selected from patients of record at Indiana University School of Dentistry. Each subject received one resin composite restoration (Tetric EvoCeram, Ivoclar Vivadent) utilizing a self-etch (SfE) universal adhesive (Adhese Universal, Ivoclar Vivadent) with no separate enamel etching and another restoration utilizing adhesive and selective enamel etching (SelE) with 37% phosphoric acid (H3PO4). Both the adhesive and composite were placed following the manufacturer’s instructions. The two techniques were compared for differences in sensitivity, retention, marginal discoloration, marginal adaptation, and clinical acceptability at baseline and 6 months using the Cochran-Mantel-Haenszel tests for stratified, ordered categorical outcomes. Seventy-four restorations (37 SfE, 37 SelE) in 30 volunteers were evaluated at 12 months. No significant differences were found between the SfE and SelE groups for any variable at the 12-month recall (p>0.21). Retention was 100% at 12 months for both groups. Marginal adaptation was significantly worse at 12 months than at baseline for SelE (p=0.0163), but there was no difference for SfE (p=0.08). Sensitivity improved significantly from baseline to 12 months for both SelE (p=0.0113) and SfE (p=0.0128). The results obtained from this study are comparable to results observed in similar studies. Like similar studies involving self-etch adhesives in non-carious cervical lesions, our study showed no restorations lost to caries and excellent retention. The deterioration of selective-etch dentin margins was a result that differed from similar studies. A likely explanation for this finding would be the difficulty of controlling precise placement of phosphoric acid gel, causing undesired etching of dentin; this could result in suboptimal bonding to dentin. This report on 12-month data for a two-year study indicates significantly reduced sensitivity for both the SelE and SfE groups, and deterioration of SelE marginal adaptation. No decreases in retention, marginal discoloration, or clinical acceptability were observed in either group.

    A clinical study of sealants polymerized with two different light sources

    This clinical study investigated the efficacy of the new LED LCU technology when compared to that of the QTH LCU by evaluating retention and wear of Clinpro (3M ESPE) sealant material over six months of function. This study was designed as a split mouth, randomized clinical study. Sealants were placed and polymerized on contralateral teeth of 35 patients, 33 of which successfully completed the study. The sealants were evaluated for clinical retention at baseline, three months, and six months by two evaluators. For the wear analysis, the area of the sealant wear at six months is reported. Nine pairs of molars and 22 pairs of premolar teeth were used. This sample size is smaller than the original sample used for clinical evaluation, because a number of the baseline impressions had to be discarded due to poor impression quality. Subsequent impressions were taken at three months, and six months. Epoxy replicas were made from the impressions and the occlusal surface of each replica was digitized using SigmaScan software. A cummulative legit model was applied to the clinical data, and a linear model was applied to the wear analysis. The results for clinical retention over the six months of function were as follows. At Baseline, for the QTH, 97.3 percent of the teeth received an Alpha score; 2.7 percent received a score of B. For the LED, 87.7 percent received a score of A; 12.3 percent received a score of B. At three months follow-up, for the QTH, 93.1 percent received a score of A; 6.9 percent received a score of B. For the LED, 86.1 percent received a score of A; 12.5 percent received a score of B, and 1.14 percent received a score of C. At six months follow-up, for QTH; 91.7 percent received a score of A; 8.3 percent received a score of B. For the LED, 83.3 percent received a score of A; 15.3 percent received a score of B, and 1.14 percent received a score of C. The hypothesis was that there would be no significant difference in clinical retention and wear of Clinpro's sealant polymerized with the QTH or the LED light sources over six months of function. Based on the results of this clinical study, the following conclusions can be made: 1) At baseline, Clinpro's sealant polymerized with QTH light source showed marginally significant better retention than LED light source (p-value 0.05001). 2) There was no significant difference between light sources for sealant clinical retention at three-month and six-month follow up visits. 3) Wear analysis resulted in marginally significant more wear for molar sealants polymerized with LED LCU (p-value 0.0755). 4) Wear analysis showed no significant difference for premolar sealants polymerized with either light source.

    Cuspal Deflection in Premolar Teeth Restored with Bulk-Fill Resin-Based Composite Materials

    Objectives: To investigate the effect of bulk-fill resin based composite materials on cuspal deflection in large slot mesio-occlusal-distal cavities (MOD) in premolar teeth. Methodology: Thirty-two sound maxillary premolar teeth with large slot MOD cavities were distributed to four groups (n=8). Three groups were restored with bulk-fill resin composite materials (Tetric EvoCeram, x-tra fil, and Sonic Fill, respectively) in a single increment. The conventional composite group, Filtek Z100, was used to restore the cavities in 2mm increments. Cusp deflection was recorded post irradiation using a Nikon measurescope UM-2 (Nikon, Tokyo, Japan), by measuring the changes in the bucco-palatal width of the premolar teeth at 5 minutes, 24 hours, and 48 hours after completion of the restoration. The cuspal deflection was obtained by recording the difference between the baseline measurements and the other measurements for each tooth. Results: Cuspal deflection was significantly higher in Conventional Composite than in Tetric EvoCeram Bulk Fill (p=0.0031), x-tra Fil Bulk (p=0.0029), and SonicFill Bulk (p=0.0002). There was no significant difference in cuspal deflection for Tetric EvoCeram Bulk, X-tra Fil Bulk, and SonicFill Bulk Composites. Conclusions: All the investigated bulk-fill resin composites exhibited cuspal deflection lower than conventional resin composite. One of the aims of research and studies on the resin composite materials is improving their clinical longevity, and simplifying their use. For that purpose bulk-fill materials are considered promising materials and further clinical studies should be conducted.

    Degradation of bleaching agents under two different storage conditions

    This study was conducted in order to determine if there is any change in the active ingredient of tooth-whitening agents when the products are received from the manufacturer (Baseline), two months, four months, and six months after they are received, under two different storage conditions. Thirty-six products were received from multiple manufacturers: eight with hydrogen peroxide and 28 carbamide with peroxide products. All the bleaching syringes for a specific product were from the same lot. Once the products were received, one sample of each product was stored at room temperature and the other sample was stored in a refrigerator. Assays to determine the baseline concentration were performed within the first two weeks of their arrival and again 2 months, 4 months, and 6 months after receiving the products. All samples were analyzed for peroxide content by using the United States Pharmacopeia recommended method. The results obtained from this study show the following: 1. Bleaching products have different concentrations than what are indicated on the label. 2. Storage of bleaching products for an extended time at room temperature can cause bleaching products to lose some of their potency. 3. The preferable storage condition verified in this study is under refrigeration.

    The effect of inhibitor and initiator concentration on degree of conversion, flexural strength and polymerization shrinkage stress on resin-matrix composite

    Polymerization shrinkage is one of the most significant problems associated with resin-matrix composite. Shrinkage results in contraction stress in the resin, leading to possible debonding in certain areas of the adhesive joint and potentially adversely affecting the bond strength. The reduction in the stress may improve the adaptation of the resin restoration, and decrease the problems that are associated with contraction stress, such as postoperative pain and recurrent caries. Recently, it has been found that varying the inhibitor concentration would reduce the polymerization shrinkage without affecting mechanical properties. In this study, we investigated the effects of varying the initiator and initiator levels on polymerization shrinkage stress, strength, and degree of conversion. An experimental composite was prepared by using a blend of BisGMA: UDMA: TEGMA (1:1:1 weight ratio) with 70 wt% silanated glass fillers. Four levels of inhibitors (BHT 0.0 %, 2%, 6%, 20%) and initiators (CQ 2%, 6%, 20%, 60%) were used (total of 16 combinations). A tensiometer was used to measure the polymerization contraction stress, contraction stress rate and gel time for each resin. FTIR was used to measure the degree of conversion. The flexural strength and flexural modulus were determined using the three-point bending test. Resin-matrix composite with 0.0-percent BHT and 2.0-percent CQ showed the highest contraction stress and stress rate and the shortest gel time, while resin-matrix composite with the 6.0-percent BHT and 6-percent CQ showed the lowest contraction stress and stress rate and the longest gel time. At an extremely high concentrations of CQ (20 percent and 60 percent) and high BHT concentration (20 percent) low degree of conversion values were observed. Overall, from the collected data, group F (2-percent BHT and 6-percent CQ) and G (6-percent BHT and 6-percent CQ) provide the most desirable combination of strength (above 80 MPa) and stress (below 3 MPa) are present as a potential dose combination range of CQ and BHT. In conclusion, the effect of inhibitors and initiators appears to change in different resin formulation. Increasing the levels of both the inhibitor and the initiator decrease the polymerization contraction stress and stress rate, and the impact on the conversion is unpredictable. In this study, we found a decrease in both the conversion value and depth of cure.

    Effect of surface conditioning methods on repair bond strength of microhybrid resin matrix composite

    Repair is an alternative treatment option in many cases to replacement of resin matrix composite restoration. However, aged resin matrix composites have a limited number of carbon-carbon double bonds to adhere to a new layer of rein. Therefore, surface treatments of the aged resin matrix composite surface prior to repairing could improve the repair bond strength. The objectives of this study were to: 1) To evaluate various surface treatments on shear bond strength of repair between aged and new microhybrid resin matrix composite, and 2) To assess the influence of applying a silane coupling agent after surface treatments. Eighty disk-shaped resin matrix composite specimens were fabricated and thermocycled 5000 times prior to surface treatment. Specimens were randomly assigned to one of the three surface treatments (n = 20): 1) Airborne abrasion with 50 μm aluminum oxide, 2) Tribochemical silica coating (CoJet), or 3) Er,Cr:YSGG laser and control group (n = 20). Specimens were cleaned with 35-percent phosphoric acid, rinsed, and dried. Each group was assigned into two subgroups (n =10): a) no silanization, and b) with silanization. Adhesive agent was applied and new resin matrix composite was bonded to each conditioned surface. Bond strength was evaluated by shear test. Data were analyzed with a two-way ANOVA model. The interaction between conditioning and silanization was significant(p = 0.0163), indicating that comparisons of silanization must be evaluated for each conditioning method, and that comparisons of conditioning methods must be evaluated separately with and without silanization. Airborne particle abrasion showed significantly higher repair bond strength than Er,Cr:YSGG laser without silanization (p < 0.0001) and with silanization(p = 0.0002), and higher repair bond strength than the control without silanization (p < 0.00001) and with silanization (p < 0.00001). Airborne particle abrasion did not have significantly different in repair bond strength than Tribosilica coating without silanization (p = 0.70) or with silanization (p = 0.33). Tribosilica coating had significantly higher repair bond strength than Er,CR:YSGG laser without silanization (p < 0.0001) and with silanization (p < 0.0001), and significantly higher repair bond strength than control without silanization (p < 0.0001), but not with silanization (p =0.16). Er,CR:YSGG laser and control did not have significantly different repair bond strength without silanization (p = 1.00) or with silanization (p = 0.11). There was no effect of silanization on repair bond strength overall (p = 0.34) for any of the surface conditioning methods (p = 0.76 for airborne particle abrasion; p = 0.39 for tribosilica coating; p = 1.00 for Er,Cr:YSGG laser, or p = 0.39 for control). Airborne particle abrasion with 50-μm aluminum oxide particle and tribochemical silica coating followed by the application of bonding agent provided the highest shear bond strength values, suggesting that they might be adequate methods to improve the quality of the repairs of resin-matrix composites.

    Effectiveness and mechanisms of action of whitening dentifrices on enamel extrinsic stains

    Whitening dentifrices use different mechanisms for stain removal and prevention. These approaches are abrasives, oxidizing agents, and chemical cleaning agents. The objectives of this in-vitro study were: 1) To compare the whitening effect of commercial whitening and non-whitening dentifrices; 2) To verify the mechanism of action of whitening dentifrices by contrasting two experimental models: chemical model (toothpaste exposure only) and chemo-mechanical (toothpaste exposure with tooth-brushing abrasion). The 256 bovine enamel specimens (10 mm x10 mm) were prepared and partially stained. They were assigned to 8 groups: 6 whitening dentifrices, 1 non-whitening reference dentifrice and deionized water (control); and they were further divided in 2 subgroups (n = 16), chemical and chemo-mechanical. Specimens were exposed to dentifrice slurries 2X/day for 1 min and brushed or not, according to each model. In between dentifrice treatments, specimens were exposed to the staining solution for 5 h. This protocol was repeated for 5 consecutive days and enamel color changes (∆E, ∆L) were measured by spectrophotometry after each day. The abrasivity of the toothpastes was determined using a standard test (ISO 11609). Significantly higher ∆E values (whitening effect) were observed for all groups (p < 0.05), except for the control, when tested in the chemo-mechanical model. In this model, the whitening ability of the toothpastes was mainly determined by their abrasive levels. For the chemical model, no significant differences were observed among groups (p > 0.05). Whitening dentifrices can be effective in preventing and removing enamel surface staining, when associated with tooth-brushing abrasion. This was modulated by the abrasive level of the tested toothpastes, with no chemical action attributed to the chemical agents.

    Evaluation of fracture resistance of three post and core systems in endodontically treated teeth under loading to failure; and marginal gap measurement before and after cyclic loading

    The purpose of this study was to evaluate the fracture resistance of three post and core systems in endodontically treated teeth by loading to failure, and to measure marginal gaps before and after cyclic loading. Sixty extracted canines were assigned to three groups. The groups tested were: 1) Single cast post and core (Group CP). 2) Prefabricated metal post and composite resin core (Group MR). 3) Glass fiber post and composite resin core (Group FR). All teeth were obturated and prepared to receive a post and core with a coping. Thirty teeth (10 from each group) were loaded to failure, and the other 30 teeth were fatigue-loaded. The marginal gaps on the facial and lingual surface of the fatigue-loaded group were measured before and after cyclic loading. There were two hypotheses for this study. The first was that the FR group would have less marginal gap opening on the lingual surface than the other groups. The second was that the CP group would have a higher load at failure than the other groups. Group CP was found to have a significantly smaller pre-loading marginal gap than group FR (p = 0.0265) and group MR (p = 0.0273), while groups FR and MR did not have a significantly different pre-loading marginal gaps (p = 0.86). Group FR had significantly less change in marginal gap than group MR (p = 0.0013). Groups CP and MR did not have significantly different changes in marginal gap (p = 0.09). Groups CP and FR did not have significantly different changes in marginal gap (p = 0.11). The three post types did not have significantly different maximum loads to failure (p= 0.49), moments of inertia at cervical area (p = 0.75), or moments of inertia at fracture site (p=0.12). There was no significant difference between groups CP, FR, and MR in the load-to-failure test. Group CP demonstrated the highest load-to-failure values; however, the highest load-to-failure mean was for group MR. All fractures observed in this study were catastrophic. Group FR demonstrated better stress distribution and caused no early fractures in the fatigue-loading group. All groups demonstrated significant marginal gap changes on the lingual surface after fatigue loading; however, group FR demonstrated <45 μm marginal gap opening. In group FR, the reduced marginal gap opening was attributed to the use of Panavia 21 with the proper surface treatments to bond to the tooth structure, the resin composite, and the metal coping.

    Evaluation of the Water Sorption and Solubility Behavior of Different Polymeric Luting Materials

    Objective: The study evaluated the water sorption (WSP) and water solubility (WSL) characteristics of different luting agents over a 180-day water storage period. Materials and Methods: Nine luting materials, i.e., conventional resin cement: Panavia F (PF), Rely X ARC (RA), self-adhesive resin cement: Rely X Unicem (RU), Breez (BZ), Maxcem Elite (MX), BisCem (BC) and resin-modified glass ionomer cement: FujiCem (FC), FujiPlus (FP) Rely X luting plus (RL) were assessed and fifty-two-disc specimens of each material were fabricated. All specimens were desiccated until a constant weight (W0) was reached. Thirteen specimens for each luting material were then randomly assigned to one of the four water immersion periods (7, 30, 90, and 180 days). After each period, the specimens were removed from the water and weighed to get W1. The samples were again desiccated for a second time and W2 was measured. Both WSP and WSL were determined by the following equations: WSP (%) = (W1 - W2) × 100/W0 and WSL (%) = (W0 - W2) × 100/W0. Assessments were performed following ISO standards. ANOVA was used to assess the effect of luting agent and time period on water sorption and solubility. Pair-wise comparisons were adjusted using Tukey's multiple comparison procedure. A significance level of 0.05 was used for all statistical tests. Results: The highest mean WSP and WSL (WSP/WSL) were demonstrated by resin-modified glass-ionomers (RL 18.32/3.25, FC 17.08/4.83, and FP 14.14/1.99), while resin luting agents showed lower WSP and WSL results (PF 1.6/0.67 and RA 1.76/0.46), respectively. The self-adhesive agents exhibited a wide range of WSP and WSL values (RU 1.86/0.13, BZ 4.66/0.93, and MX 3.68/1.11). Self-adhesive cement showed lower WSP and WSL compared with the resin-modified glass-ionomers (p < 0.05). All the materials reached equilibrium after 90-days. Conclusions: Resin-based luting materials have the lowest sorption and solubility. Rely X Unicem self-adhesive luting materials were comparable to resin luting materials for WSL and WSP. Resin-modified glass-ionomer showed the highest water sorption and solubility compared with both resin and self-adhesive materials.
