Item The effect of acid etching on remineralization of incipient caries lesions : a micro-ct study(2009) Yeslam, Hanin E.; Ando, Masatoshi; Gonzalez-Cabezas, Carlos, 1966-; Chu, Tien-Min Gabriel; Lund, Melvin; Cochran, MichaelEtching of enamel caries lesions has been demonstrated to enhance remineralization. However, this effect reaches a plateau after a period of time. This study aimed at investigating the effectiveness of additional acid etching on remineralization. Forty 1 mm × 2 mm human enamel blocks with chemically induced artificial incipient lesions were used. Ten specimens were randomly selected at the end of demineralization for transverse microradiography (TMR) analysis. The remaining specimens were then divided into three groups (n = 10). Group A was remineralized by a pH cycling system with 1100 ppm sodium fluoride for 20 days. In group B, the specimens were etched with 35-percent phosphoric acid for 30 s and then remineralized. Group C was remineralized by same procedure as group B plus and given an additional acid etch after 10 days of remineralization. Mineral density was measured by x-ray microtomography (µ-CT). The volumetric mineral content [VM (µm3×105)] was determined between 91 and 0-wt%. The µ-CT % mineral recovery (%) was calculated using the formula 100×(remineralize VM - demineralization VM) / (sound VM - demineralization VM). One-hundred-μm sections of demineralized and remineralized specimens were used to assess the mineral loss (IML: vol%×µm) and lesion depth (µm) using TMR. The three groups showed no significant difference in mineral change or mineral content for µ-CT or TMR lesion depth. The TMR IML showed a significant difference between the demineralized specimens and the three remineralized groups. The correlation between TMR IML and TMR lesion depth was 0.66 (p < 0.0001). The µ-CT percent mineral recovery from demineralization was correlated with neither TMR IML nor TMR lesion depth. When evaluated with µ-CT, the twice-acid-etched group presented lower mineral gain values than the group etched only once with acid. Also, the twice-etched group presented lower mineral gain and greater TMR IML compared with the non-acid etch group. TMR images revealed reduction of surface layer in the acid-etched groups, especially in the twice-etched group, in which significant reduction or loss of surface layer occurred. Based on these results, we conclude that additional acid etching with 35-percent phosphoric acid does not enhance remineralization compared with a single application of acid etching. We believe that the viable existence of the surface layer is essential for remineralization of the lesion. Further investigations into the accuracy of µ-CT to detect minute mineral changes in incipient caries lesions are probably needed.Item Effect of surface conditioning methods on repair bond strength of microhybrid resin matrix composite(2010) Rajitrangson, Phitakphong, 1982-; Cochran, Michael A. (Michael Alan), 1944-; Cho, Sopanis D.; Gonzalez-Cabezas, Carlos, 1966-; Matis, Bruce A.; Platt, Jeffrey A., 1958-Repair is an alternative treatment option in many cases to replacement of resin matrix composite restoration. However, aged resin matrix composites have a limited number of carbon-carbon double bonds to adhere to a new layer of rein. Therefore, surface treatments of the aged resin matrix composite surface prior to repairing could improve the repair bond strength. The objectives of this study were to: 1) To evaluate various surface treatments on shear bond strength of repair between aged and new microhybrid resin matrix composite, and 2) To assess the influence of applying a silane coupling agent after surface treatments. Eighty disk-shaped resin matrix composite specimens were fabricated and thermocycled 5000 times prior to surface treatment. Specimens were randomly assigned to one of the three surface treatments (n = 20): 1) Airborne abrasion with 50 μm aluminum oxide, 2) Tribochemical silica coating (CoJet), or 3) Er,Cr:YSGG laser and control group (n = 20). Specimens were cleaned with 35-percent phosphoric acid, rinsed, and dried. Each group was assigned into two subgroups (n =10): a) no silanization, and b) with silanization. Adhesive agent was applied and new resin matrix composite was bonded to each conditioned surface. Bond strength was evaluated by shear test. Data were analyzed with a two-way ANOVA model. The interaction between conditioning and silanization was significant(p = 0.0163), indicating that comparisons of silanization must be evaluated for each conditioning method, and that comparisons of conditioning methods must be evaluated separately with and without silanization. Airborne particle abrasion showed significantly higher repair bond strength than Er,Cr:YSGG laser without silanization (p < 0.0001) and with silanization(p = 0.0002), and higher repair bond strength than the control without silanization (p < 0.00001) and with silanization (p < 0.00001). Airborne particle abrasion did not have significantly different in repair bond strength than Tribosilica coating without silanization (p = 0.70) or with silanization (p = 0.33). Tribosilica coating had significantly higher repair bond strength than Er,CR:YSGG laser without silanization (p < 0.0001) and with silanization (p < 0.0001), and significantly higher repair bond strength than control without silanization (p < 0.0001), but not with silanization (p =0.16). Er,CR:YSGG laser and control did not have significantly different repair bond strength without silanization (p = 1.00) or with silanization (p = 0.11). There was no effect of silanization on repair bond strength overall (p = 0.34) for any of the surface conditioning methods (p = 0.76 for airborne particle abrasion; p = 0.39 for tribosilica coating; p = 1.00 for Er,Cr:YSGG laser, or p = 0.39 for control). Airborne particle abrasion with 50-μm aluminum oxide particle and tribochemical silica coating followed by the application of bonding agent provided the highest shear bond strength values, suggesting that they might be adequate methods to improve the quality of the repairs of resin-matrix composites.Item Flexural strength and shear bond strength of self-etching/self-adhesive resin luting agents(2009) Adcook, Richard S.; Cochran, Michael A.; Platt, Jeffrey A.; Gonzalez-Cabezas, Carlos, 1966-; Matis, Bruce A.; Lund, Melvin R.Traditional resin luting agents generally have mechanical properties that are superior to the newer so-called “universal” self-etching/self-adhesive resin luting agents. However, recent reports indicate that some properties of these new luting agents have been improved, approaching those of the traditional etch and rinse resin luting agents. The objective of this study was to test some mechanical properties of four of these self-etching/self-adhesive resin luting agents [Maxcem Elite (ME), Multilink Automix (MA), RelyX Unicem (RU), SmartCem 2 (SC)] and compare them to a traditional etch and rinse resin luting agent [RelyX ARC (RA)] and a resin-modified glass ionomer luting cement [Fuji Plus (FP)], both of which have much longer histories of clinical success. By comparing the properties of the newer cements to the standards, it may be possible to determine how clinically successful the newer cements may be. The mechanical properties tested were flexural strength (FS) and shear bond strength (SBS). The FS test included making beams of each material, storing them in water for periods of time (24 hours and 90 days) and then performing a three-point bending test on a universal testing machine. The 90 day groups were thermocycled. The SBS test involved preparing human molar specimens, making flat dentin surfaces. Composite cylinders were fabricated, luted to the dentin surfaces with each of the materials tested, stored in water for periods of time (24 hours or 90 days), and then a knife edge shear test was performed on a universal testing machine. The 90 day groups were thermocycled. A Weibull-distribution survival analysis was performed. The results revealed significant differences in the FS of all materials tested at 24 hours. After 90 days and thermocycling, only SC and RA were not significantly different. At both time periods, FP had the lowest and MA the highest FS. The SBS results showed MA, RA, and RU to have the highest bond strengths; SC and ME the lowest at 24 hours. After 90 days and thermocycling, RA had significantly higher bond strength than all other groups; ME, FP and SC had the lowest. The self-etching/self-adhesive resin luting agents all performed at least as well as FP, with the exception of SC (SBS 24 hour). They did not all perform as well as RA, with the exception of SC (FS 90 day), MA (SBS 24 hour, FS 24 hour and 90 day), and RU (SBS 24 hour). The newer luting agents should expect to have clinical success, regarding flexural strength and shear bond strength, at least as good as resin-modified glass ionomer luting cements and approach the level of traditional etch and rinse resin luting agents.Item The genetic susceptibility/resistance to fluorosis among different inbred mouse strains(2003) McHenry, Melissa A.K., 1971-; Everett, Eric T.; Dean, Jeffrey A.; Gonzalez-Cabezas, Carlos, 1966-; Jackson, Richard D.; Sanders, Brian J.; Stookey, George K.Fluoridation of community water supplies for the purpose of preventing dental caries remains one of the top 10 public health interventions of the last century. However, exposure (ingestion) of greater than optimal amounts of fluoride from a variety of sources has led to an increase in the prevalence of dental fluorosis. We propose that dental fluorosis represents a complex condition caused by environmental and genetic factors. Purpose: To assess the role of genetics in the pathogenesis of dental fluorosis using genetically separate inbred strains of mice. Methods: Twelve genealogically disparate strains of mice were treated with 0 ppm, 25 ppm, and 50 ppm of fluoride in their drinking water. Each mouse was given weekly dental fluorosis evaluations. After 60 days of treatment, femurs were collected for fluoride analysis. Mandibular incisors were isolated for quantitative light induced fluorescence (QLF) studies and fluoride analysis. Digital and 35 mm images were taken of all mouse incisors in order to apply and compare the Dean's Index and the modified Thylstrup and Fejerskov Index (TFI), both indices of dental fluorosis. Skeletal radiographs were taken on the euthanized mice and later examined for extra skeletal calcifications and other gross bony deformities. Results: Differences in dental fluorosis susceptibility/resistance were identified between the strains, ranging from mild, moderate, to severe dental fluorosis. Furthermore, we found clustering of strains into distinct phenotypic groups. The A/J mouse strain was highly susceptible, with a rapid onset and severe development of dental fluorosis compared with the other strains tested. The 129P3/J mouse strain was least affected with negligible dental fluorosis. From the skeletal radiographs, no gross skeletal lesions or evidence of bone dysplasia were noted. Similar body burden of fluoride, as judged from analysis of mineralized tissues, was seen in all strains despite differences in their predispositions to develop dental fluorosis. Both the Dean's and TF indices are useful for classifying the stage or severity of fluorosis in mice, and there are advantages to the use of digital images over conventional 35 mm slide images. Both indices correlate well with the amount of fluoride exposure during amelogenesis; however, these indices are not promising indicators of fluoride burden during amelogenesis. Conclusions: QLF proved to be an innovative and useful tool for the quantification of dental fluorosis. Furthermore, these observations support the role of a genetic component in the pathogenesis of fluorosis.Item The Immunogenetics of Dental Caries(2010) McCarlie, Van Wallace, Jr.; Gregory, Richard L.; Fontana, Margherita Ruth, 1966-; Hartsfield, James K., Jr.; Blum, Janice S.; Gonzalez-Cabezas, Carlos, 1966-; Chin, Judith R.Background: Bacterial adherence to the acquired dental pellicle, important in caries, is mediated by receptor-adhesin interactions such as Streptococcus mutans antigen I/II (I/II). Ten I/II epitopes from the A, V, P and C regions were chosen to determine their reactivity in human saliva. Underlying the body’s ability to immunologically respond to bacteria that lead to caries are the human leukocyte antigen (HLA) genes, specifically HLA class II (HLA-II) genes that control antigen presentation. Previous studies suggested that a specific HLA biomarker group (HLA-DRB1*04) may have differential control of immune responses to I/II. However, it was not known whether secretory IgA (SIgA) responses to the selected epitopes from HLA-DRB1*04 positive subjects were different compared to their non-biomarker counterparts (negative), or across other caries factors, since no study to date had thus assessed these questions. Methods: Per IRB approval, the study population was divided into age, sex and race matched DRB1*04 positive (n=16) and negative groups (n=16). SIgA-epitope (and whole cell) reactivity was determined using ELISA. Other caries factors were measured. Subjects received a clinical exam by a trained examiner. ix Differences between DRB1*04 positive and negative groups were examined using a two-sided, two-sample t-test. Results: DRB1*04 positive subjects had numerically, but not statistically, higher reactivity to 9 out of 10 epitopes, the exception being residues 834-853 from the V and P regions of I/II across multiple measures. Though statistically insignificant, DRB1*04 positive subjects also exhibited 25-30 μg mL-1 less total IgA (TIgA) than negative counterparts. All clinical caries data proved inconclusive when comparing groups, likely due to exogenous factors and sample size. Conclusion: DRB1*04 positive subjects showed a trend toward lower TIgA. Moreover, they also showed a lower SIgA response across multiple measures to 834-853, the I/II V and P region epitope. This region forms a sort of functional epicenter involved in collaboration between domains along the entire I/II antigen, and governs the region involved in initial attachment to the acquired dental pellicle. This region may be involved in an in vivo discontinuous conformationally specific immunogenic epitope that serves as an HLA-II binding motif which remains elusive.
