This document outlines the standardized data elements included in Indiana’s 2026 Cross-Profession Minimum Data Set (CPMDS) license renewal survey, adapted from the national CPMDS framework. It captures essential demographic, educational, licensure, employment, and practice location information across licensed health professions. The survey also includes questions on telehealth usage, populations served, and hours worked in direct patient care.
