Abstract
This document contains three population-specific mixed-method surveys that were used as data collection tools, from July 15th, 2024-October 1, 2024, for a cancer health equity study. The goal of this study was to assess different factors that impact the quality of cancer care and outcomes from the perspective of patients, their support members, and clinicians in Central Indiana. Each survey consisted of standard demographic questions, items from the Cancer Awareness Measure (CAM), and the Coping Orientation to Problems Experienced (COPE) Inventory. The clinician survey contained additional questions examining their level of exposure to formal training on inclusive practices to promote equitable cancer care delivery.
