Abstract

Dr. Lacey uses a running series of workplace fatality case studies called Fatality Investigation Reports that are produced by the Washington State Department of Labor & Industries as a centerpiece in his classes. These public domain case studies are packaged as PDFs with narrative descriptions of the incident, along with figures, schematics, and photos.
