Item Change in self-efficacy partially mediates the effects of the FRESH START intervention on cancer survivors' dietary outcomes(Wiley, 2008) Mosher, Catherine E.; Fuemmeler, Bernard F.; Sloane, Richard; Kraus, William E.; Lobach, David F.; Snyder, Denise Clutter; Demark-Wahnefried, WendyObjective: This study examined change in self-efficacy as a mediator of the effects of a mailed print intervention on the dietary and exercise practices of newly diagnosed breast and prostate cancer survivors. Method: A total of 543 breast and prostate cancer patients were recruited from 39 states and two provinces within North America. Participants were randomly assigned to receive a 10-month program of tailored mailed print materials that aimed to increase fruit and vegetable consumption, reduce fat intake, and/or increase exercise or a 10-month program of publically available materials on diet and exercise. Telephone surveys conducted at baseline and 1 year assessed dietary practices, physical activity, and self-efficacy for engaging in these health behaviors. Results: Results indicated that changes in self-efficacy for fat restriction and eating more fruits and vegetables were significant mediators of the intervention's effects on dietary outcomes at 1-year follow-up. The intervention did not significantly affect self-efficacy for exercise; however, a significant, positive relationship was found between self-efficacy for exercise and exercise duration at follow-up. Conclusions: Findings are largely consistent with Social Cognitive Theory and support the use of strategies to increase self-efficacy in health promotion interventions for cancer survivors. Materials and methods The study included a sample of 719 individuals from 51 archeological sites (450 farmers, 192 foragers, 77 pastoralists). All were over age 12 and sexes were pooled. Using a Sensofar® white‐light confocal profiler we collected dental microwear texture analysis (DMTA) data from a single first or second molar from each individual. We leveled and cleaned data clouds following standard procedures and analyzed the data with Sfrax® and Toothfrax® software. The DMTA variables were complexity and anisotropy. Statistics included ANOVA with partial eta squared and Hedges's g. We also performed a follow‐up K‐means cluster analysis. Results We found significant differences between foragers and farmers and pastoralists for complexity and anisotropy, with foragers having greater complexity than either the farmers or the pastoralists. The farmers and pastoralists had greater anisotropy than the foragers. The Old World foragers had significantly higher anisotropy values than New World foragers. Old and New World farmers did not differ. Among the Old World farmers, those dating from the Neolithic through the Late Bronze Age had higher complexity values than those from the Iron Age through the medieval period. The cluster analysis discerned foragers and farmers but also indicated similarity between hard food foragers and hard food farmers. Discussion Our findings reaffirm that DMTA is capable of distinguishing human diets. We found that foragers and farmers, in particular, differ in their microwear signatures across the globe. There are some exceptions, but nothing that would be unexpected given the range of human diets and food preparation techniques. This study indicates that in general DMTA is an efficacious means of paleodietary reconstruction in humans.Item Depressive Symptoms and Weight Loss Behaviors in U.S. Adults(Elsevier, 2018) Vrany, Elizabeth A.; Hawkins, Misty A. W.; Wu, Wei; Stewart, Jesse C.; Psychology, School of ScienceObjective We sought to determine whether depressive symptoms are associated with attempting to lose weight and engaging in weight loss behaviors in a large, diverse sample of adults representative of the U.S. population. Methods Respondents were 23,106 adults, free of cardiovascular disease and diabetes, who participated in the 2005–2014 years of the National Health and Nutrition Examination Survey (NHANES). Depressive symptoms were measured using the Patient Health Questionnaire-9 (PHQ-9), and weight loss variables were obtained from a Weight History Questionnaire. Results PHQ-9 total was not associated with attempting to lose weight in the past year (OR = 1.03, 95%CI = 1.00–1.06, p = 0.074; n = 23,106). Among respondents who attempted to lose weight (n = 9582), PHQ-9 total was associated with a lower odds of exercising (OR = 0.84, 95%CI = 0.79–0.89, p < 0.001) and a greater odds of skipping meals (OR = 1.31, 95%CI = 1.22–1.41, p < 0.001), eating diet foods/products (OR = 1.16, 95%CI = 1.08–1.24, p < 0.001), eating less food (OR = 1.09, 95%CI = 1.04–1.15, p < 0.001), taking non-prescription supplements (OR = 1.31, 95%CI = 1.23–1.41, p < 0.001), taking prescription diet pills (OR = 1.28, 95%CI = 1.10–1.49, p = 0.001), and taking laxatives/vomiting (OR = 1.55, 95%CI = 1.28–1.88, p < 0.001). Conclusions Although depressive symptoms were not associated with attempting to lose weight in the past year, adults who attempted to lose weight tended to employ potentially ineffective/unhealthy weight loss behaviors and avoid effective behaviors. This pattern of behaviors may be another mechanism that explains the excess risk of obesity in depressed adults and may be a modifiable target for future interventions. Given the cross-sectional nature of this study, reverse causality is a possibility. Future studies should investigate the prospective associations between depressive symptoms and weight loss behaviors.Item DHA Alters Raft-like Membrane Domains as Revealed by Solid State 2H NMR Spectroscopy(Office of the Vice Chancellor for Research, 2015-04-17) Kinnun, Jacob J.; Williams, Justin A.; Stillwell, William; Bittman, Robert; Shaikh, Saame Raza; Wassall, Stephen R.Dietary omega-3 polyunsaturated fatty acids (n-3 PUFAs), such as docosahexaenoic acid (DHA, 22:6), are correlated with the prevention of neurological and autoimmune disorders in humans. These fatty acids must be obtained from the diet, such as oil fish or fish oil supplements, as they cannot be generated within the human body. The origin of the health benefits at the molecular level is still under question. A membrane-mediated mechanism in which n-3 PUFAs are incorporated into phospholipids and modulate molecular organization is one possibility. Cellular membranes are inhomogeneous where structurally diverse lipids can exist in separate domains. Regions rich in sphingomyelin (SM) and cholesterol, commonly called lipid rafts, contain important signaling proteins. In a recent solid-state 2H nuclear magnetic resonance (2H NMR) study of a model membrane composed of 1-[2H31] palmitoyl-2-docosahexaenoyl-phosphatidylcholine (PDPC-d31), a deuterated analog of a DHA-containing phospholipid, in mixtures with SM and cholesterol, we discovered that DHA could significantly enter raft-like domains. How DHA affects the molecular organization within the raft-like domains is addressed here by observing PSM-d31, an analog of SM with a perdeuterated N-palmitoyl chain. The 2H NMR spectra for PSM-d31, in mixtures with PDPC and cholesterol, exhibit two spectral components, a larger more ordered component that we attribute to raft-like domains and a smaller less ordered component that we attribute to non-raft-like domains. On average, the order of PSM-d31 is reduced and, thus, disordering of PSM-d31 by PDPC is indicated. Our observations confirm that DHA can infiltrate rafts and affect molecular organization, which has implications for the signaling of raft and non-raft proteins. Furthermore, these results are consistent with in vivo studies showing that DHA infiltrates rafts.Item Diet composition as a candidate mechanism underlying the depression-to-obesity association: the CARDIA study(2018) Vrany, Elizabeth A.; Stewart, Jesse C.People with depression are at elevated risk for future obesity; however, little is known about the potential mechanistic role of diet composition in this association. The aims of the present study were: (1) to examine depressive symptom severity as a predictor of 13-year change in seven diet composition factors over time, (2) to test 13-year change in diet composition factors as mediators of the association between depressive symptom severity and 13-year change in adiposity, and (3) to explore whether the Aim 2 mediation models are moderated by sex and race. Participants were 2,449 non-Hispanic Black and White adults who participated in the 1990, 1992, and 2005 years of the CARDIA study (mean baseline age = 35 years, 54% women, 56% non-Hispanic Black, mean baseline waist circumference = 84.0 cm, mean change in waist circumference = 8.3 cm). Depressive symptoms were assessed in 1990 using the Center for Epidemiologic Studies-Depression Scale (CES-D). Waist circumference and seven diet composition factors were assessed in 1992 and 2005. Diet composition factors included intake of total energy, saturated fatty acid, monounsaturated fatty acid, polyunsaturated fatty acid, fiber, sugar, and protein. PROCESS bootstrapping analyses were used to test for mediation and moderated mediation (see Figure 1 for the conceptual model). All models included adjustment for age; sex; race; education; prevalent and incident cardiovascular disease, diabetes, and cancer; incident pregnancy; and 13-year change in physical activity. Greater baseline CES-D Total predicted greater 13-year increases in waist circumference (path c β = 0.032, p = .006). In separate models, there was a trend for greater baseline depressive symptoms predicting 13-year increases in total energy intake (path a β = 0.040, p = .054), and greater baseline depressive symptoms predicted 13-year increases in protein intake (path a β = 0.059, p = .004) and fiber intake (path a β = 0.040, p = .0496). Depressive symptoms were unrelated to 13-year changes in intake of the other diet composition factors of saturated fatty acid, monounsaturated fatty acid, polyunsaturated fatty acid, and sugar (all ps ≥ .129). Mediation models revealed that 13-year change in total energy intake and protein intake partially mediated the association between baseline depressive symptoms and 13-year change in waist circumference (total energy intake indirect effect = 0.001, 95% CI=0.0001-0.004; protein intake indirect effect = 0.002, 95% CI=0.0004-0.005); mediation was not observed for the other diet composition factors. Exploratory moderated mediation models detected a significant interaction for CES-D Total by sex predicting 13-year change protein intake only (Index of moderated mediation=0.004, 95%CI=0.001-0.010). All other tests of moderated mediation were non-significant (95% CIs included zero). Analyses stratified by sex revealed that CES-D Total was more strongly associated with 13-year increases protein intake in men (β = 0.011, p=.004) than in women (β = 0.027, p = .246). Findings from this larger, longitudinal, epidemiologic study suggest that the prospective relationship between depression and future obesity risk may be explained, in part, by intake of certain diet composition factors. Advancing the understanding of the relationship between depression and diet changes will inform clinical efforts to prevent or manage depression-related obesity.Item Effects of diet and exercise on weight-related outcomes for breast cancer survivors and their adult daughters: an analysis of the DAMES trial(Springer, 2017-08) Tometich, Danielle B.; Mosher, Catherine E.; Winger, Joseph G.; Badr, Hoda J.; Snyder, Denise C.; Sloane, Richard J.; Demark-Wahnefried, Wendy; Department of Psychology, School of SciencePurpose Few trials have aimed to promote diet and exercise behaviors in both cancer survivors and their family members and examine their associations with weight-related outcomes. We conducted a secondary analysis to examine associations between change in diet and exercise behaviors and weight-related outcomes for overweight breast cancer survivors and their overweight adult daughters in the Daughters And MothErS Against Breast Cancer (DAMES) randomized trial. Methods The DAMES trial assessed the impact of two iteratively tailored, mailed print diet and exercise interventions against standard brochures over a 12-month period. This analysis examined change in diet and exercise behaviors and weight-related variables from baseline to post-intervention for the 50 breast cancer survivors and their adult daughters randomized to the intervention arms. To reduce the potential for type II error in this pilot, p values <0.10 were considered statistically significant. Results For mothers, change in diet quality was uniquely related to change in BMI (β = −0.12, p = 0.082), weight (β = −0.12, p = 0.060), and waist circumference (β = −0.38, p = 0.001), whereas change in caloric intake was related to waist circumference (β = 0.21, p = 0.002). For daughters, change in caloric intake was related to change in waist circumference (β = 0.12, p = 0.055). However, change in diet quality was not associated with weight-related outcomes in daughters. Additionally, change in exercise was not associated with weight-related outcomes in mothers or daughters. Conclusions Findings support mail-based and other tailored interventions for weight loss in this population, with an emphasis on diet quality for breast cancer survivors and caloric intake for their adult daughters.Item An Epidemiological Review of Diet and Cutaneous Malignant Melanoma(AACR, 2018-10) Yang, Keming; Fung, Teresa T.; Nan, Hongmei; Epidemiology, School of Public HealthIncidence of cutaneous malignant melanoma has continued to rise despite public efforts to promote sun protection behaviors among populations at risk. However, dietary factors may also affect the development of melanoma. In the past few decades, findings from epidemiologic and experimental research have linked consumption of several foods and other nutrients to the risk of melanoma. Caffeine has been associated with a lower risk of melanoma, and citrus fruits and alcohol with increased risk. Associations between polyunsaturated fatty acid, niacin/nicotinamide, folate, and vitamin D with melanoma remain controversial. Diet likely influences melanoma development through several potential mechanisms, such as enhancing UV-induced apoptosis and increasing photosensitivity. We conducted a narrative review to summarize recent epidemiologic studies of diet and melanoma based on published literature. Given the high prevalence of the food items and nutrients covered in this review and the decades-long rising melanoma incidence worldwide, the associations we discuss may have important public health implications in terms of reducing melanoma incidence through dietary modification.Item A high-fat diet catalyzes progression to hyperglycemia in mice with selective impairment of insulin action in Glut4-expressing tissues(Elsevier, 2022-01) Reilly, Austin M.; Yan, Shijun; Huang, Menghao; Abhyankar, Surabhi D.; Conley, Jason M.; Bone, Robert N.; Stull, Natalie D.; Horan, Daniel J.; Roh, Hyun C.; Robling, Alexander G.; Ericsson, Aaron C.; Dong, Xiaocheng C.; Evans-Molina, Carmella; Ren, Hongxia; Pediatrics, School of MedicineInsulin resistance impairs postprandial glucose uptake through glucose transporter type 4 (GLUT4) and is the primary defect preceding type 2 diabetes. We previously generated an insulin-resistant mouse model with human GLUT4 promoter-driven insulin receptor knockout (GIRKO) in the muscle, adipose, and neuronal subpopulations. However, the rate of diabetes in GIRKO mice remained low prior to 6 months of age on normal chow diet (NCD), suggesting that additional factors/mechanisms are responsible for adverse metabolic effects driving the ultimate progression of overt diabetes. In this study, we characterized the metabolic phenotypes of the adult GIRKO mice acutely switched to high-fat diet (HFD) feeding in order to identify additional metabolic challenges required for disease progression. Distinct from other diet-induced obesity (DIO) and genetic models (e.g., db/db mice), GIRKO mice remained leaner on HFD feeding, but developed other cardinal features of insulin resistance syndrome. GIRKO mice rapidly developed hyperglycemia despite compensatory increases in β-cell mass and hyperinsulinemia. Furthermore, GIRKO mice also had impaired oral glucose tolerance and a limited glucose-lowering benefit from exendin-4, suggesting that the blunted incretin effect contributed to hyperglycemia. Secondly, GIRKO mice manifested severe dyslipidemia while on HFD due to elevated hepatic lipid secretion, serum triglyceride concentration, and lipid droplet accumulation in hepatocytes. Thirdly, GIRKO mice on HFD had increased inflammatory cues in the gut, which were associated with the HFD-induced microbiome alterations and increased serum lipopolysaccharide (LPS). In conclusion, our studies identified important gene/diet interactions contributing to diabetes progression, which might be leveraged to develop more efficacious therapies.Item High-quality diet, physical activity, and college education are associated with low risk of NAFLD among the US population(Wiley, 2021) Vilar‐Gomez, Eduardo; Nephew, Lauren D.; Vuppalanchi, Raj; Gawrieh, Samer; Mladenovic, Andrea; Pike, Francis; Samala, Niharika; Chalasani, Naga; Medicine, School of MedicineBackground and Aims The effects of diet quality (DQ), physical activity (PA), and socioeconomic status (SES) on the risk of NAFLD are unclear. We examined the association among DQ, PA, SES, and NAFLD risk. Approach and Results This is a cross-sectional analysis of the National Health and Nutrition Examination Surveys, 2017–2018, which included 3589 participants with reliable information on vibration-controlled transient elastography (VCTE) measurements, 24-h dietary recalls, PA, and SES. DQ was assessed by the Healthy Eating Index (HEI)-2015. PA was determined by the Global Physical Activity Questionnaire. SES was assessed by the educational attainment and family poverty income ratio (PIR). Risk of NAFLD was considered by means of a composite outcome using VCTE measurements: non-NAFLD versus NAFLD without clinically significant fibrosis (CSF) versus NAFLD with CSF. The NAFLD risk was lower in physically active (≥600 metabolic equivalent of task [MET] min/week) versus inactive participants (<600 MET min/week) (OR: 0.71, p = 0.043). A high-quality diet (HQD) (HEI > 56.64) was associated with a lower risk of NAFLD (OR: 0.58, p < 0.01) compared with a non-HQD. The lowest NAFLD risk was observed in those physically active with HQD (OR: 0.43, p < 0.01). Body mass index and waist circumference significantly mediated the effect of DQ and PA on NAFLD risk. Education (college or above) (OR: 0.65, p = 0.034), but not PIR, was associated with a reduced NAFLD risk. HQD and increased PA partially mediated the effect of education on NAFLD risk. The total effect of education on NAFLD risk mediated by DQ was 29% and by PA was 8%. Conclusions HQD, increased physical activity, and college education were associated with lower NAFLD risk in the US population.Item Investigation of the Cultural Context of Sugars Consumption Behavior in Low-Income Mexican-American Women(Digital Scholarship@UNLV, 2017) Benitez, Tanya J.; Keller, Colleen; Coe, Kathryn; Tasevska, Natasha; Social and Behavioral Sciences, School of Public HealthDespite the recent federal dietary recommendations to limit consumption of added sugars to less than ten percent of daily caloric intake, there is a gap in published literature examining the influence of food preparation behaviors on sugars consumption among low-income Mexican-American women. The purpose of the study was to describe the cultural context of Mexican-American women in procuring, preparing and presenting added sugars in their families’ diets. Five focus groups were conducted to examine sugars consumption behavior in thirteen overweight/obese low-income Mexican-American women ages 27-40 years. Themes that emerged during the sessions included: changes in food procurement to include high-sugar foods and sugar-sweetened beverages following migration from Mexico to the U.S.; children’s influence on what was bought and consumed in the household; changes in household diet when relatives prepared food; and influence of family traditions/extensive social gatherings and traditional foods during holidays on sugars consumption. Culturally relevant factors influencing sugars consumption were identified. We report strategies that can be used in public health interventions to reduce sugars intake among low-income Mexican-American women; such strategies must acknowledge cultural and contextual factors of social ties, the role of family members in influencing diet, and importance of maintaining traditional foods and cultural celebrations.
