Item A descriptive study of the multidisciplinary healthcare experiences of inpatient resuscitation events(Elsevier, 2023-01-06) Varner-Perez, Shelley E.; Shelley E., Kelly A. L.; Banks, Sarah K.; Burke, Emily S.; Slaven, James E.; Morse, Gregory J.; Whitaker, Myra K.; Cottingham, Ann H.; Ahmed, Rami A.; Biostatistics, School of Public HealthBackground: In-hospital resuscitation events have complex and enduring effects on clinicians, with implications for job satisfaction, performance, and burnout. Ethically ambiguous cases are associated with increased moral distress. We aim to quantitatively describe the multidisciplinary resuscitation experience. Methods: Multidisciplinary in-hospital healthcare professionals at an adult academic health center in the Midwestern United States completed surveys one and six weeks after a resuscitation event. Surveys included demographic data, task load (NASA-TLX), overall and moral distress, anxiety, depression, and spiritual peace. Spearman's rank correlation was computed to assess task load and distress. Results: During the 5-month study period, the study included 12 resuscitation events across six inpatient units. Of 82 in-hospital healthcare professionals eligible for recruitment, 44 (53.7%) completed the one-week post-resuscitation event survey. Of those, 37 (84.1%) completed the six-week survey. Highest median task load burden at one week was seen for temporal demand, effort, and mental demand. Median moral distress scores were low, while "at peace" median scores tended to be high. There were no significant non-zero changes in task load or distress scores from weeks 1-6. Mental demand (r = 0.545, p < 0.001), physical demand (r = 0.464, p = 0.005), performance (r = -0.539, p < 0.001), and frustration (r = 0.545, p < 0.001) significantly correlated with overall distress. Performance (r = -0.371, p = 0.028) and frustration (r = 0.480, p = 0.004) also significantly correlated with moral distress. Conclusions: In-hospital healthcare professionals' experiences of resuscitation events are varied and complex. Objectives: To determine the relationships between dimensions of religion and spirituality and medical treatment decisions made by surrogates. Methods: This prospective observational study enrolled patient/surrogate dyads from three hospitals in one metropolitan area. Eligible patients were 65 years or older and admitted to the medicine or medical intensive care services. Baseline surveys between hospital days 2 and 10 assessed seven dimensions of religion and spirituality. Chart reviews of the electronic medical record and regional health information exchange six months after enrollment identified the use of life-sustaining treatments and hospice for patients who died. Results: There were 291 patient/surrogate dyads. When adjusting for other religious dimensions, demographic, and illness factors, only surrogates' belief in miracles was significantly associated with a lower surrogate preference for do-not-resuscitate status (adjusted odds ratio [aOR] 0.39; 95% CI 0.19, 0.78). Among patients who died, higher surrogate intrinsic religiosity was associated with lower patient receipt of life-sustaining treatments within the last 30 days (aOR 0.66; 95% CI 0.45, 0.97). Belief in miracles (aOR 0.30; 95% CI 0.10, 0.96) and higher intrinsic religiosity (aOR 0.70; 95% CI 0.53, 0.93) were associated with lower hospice utilization. Conclusion: Few religious variables are associated with end-of-life preferences or treatment. Belief in miracles and intrinsic religiosity may affect treatment and should be identified and explored with surrogates by trained chaplains or other clinicians with appropriate training.Item Because I Am Human: Centering Black Women with Dis/abilities in Transition Planning from High School to College(2019-02) Cannon, Mercedes Adell; Thorius, Kathleen; Thompson, Chalmer; Mutegi, Jomo; Rogan, Patricia; Skelton, SeenaThere is a dearth of literature about post-secondary transition experiences of Black women with dis/abilities (BWD). In this qualitative study, I explore transition experiences of five post-secondary BWD from high school to college in order to privilege her chronicles and narratives as knowledge. In addition, two urban public high school transition coordinators (TC) participated in the study. Three inquiries guided my dissertation: (1) features of educational experiences narrated by BWD, (2) features of transition services provided to students with dis/abilities, including roles of and approaches as described by the TCs, and (3) how BWD narratives may be leveraged to critique and extend transition services as the TCs described them. I engaged in three semi-structured interviews with six of the seven participants (one interview with the seventh). I drew from Disability Studies/Disability Studies in Education (DSE), Critical Race Theory, and Womanist/Black Feminist Theory and their shared tenets of voice and counternarratives and concepts of social construction and falsification of consciousness to analyze the narratives of BWD participants. I drew from the DS/DSE tenet of interlocking systems of oppression, DisCrit tenet three, race and ability, and constructs of Inputs and Outcomes in work on Modeling Transition Education to analyze the TCs’ narratives and in connection to the narratives of the BWD. Across both sets of participants, three themes in the form of Truths emerged; they were terrible and sticky experiences of racial/dis/ability oppression for the BWDs and, imposing of whiteness and normalization within the transition education practices described by the TCs. For the BWD, those terrible and sticky truths took three forms: (a) Pathologization; (b) Disablement; and (c) Exclusion. Another type of truth in the BWD’s narratives, however, was Subverted Truths: (re)defined identities and radical love, (re)placed competence and knowledge, and (revalued sisterhood and community, the ways of pushing back and resisting the Truths and their effects. I discuss implications for BWD post-secondary transition-planning-and-programming theory, research, policy, practice, praxis, and spirituality.Item CLAIMING A SACRED FACE: A QUALITATIVE STUDY OF THE ROLE OF SPIRITUAL EXPERIENCE IN CLAIMING A POSITIVE CULTURAL IDENTITY(2005-09-27T17:44:53Z) Tisdell, Elizabeth J.This paper discusses the results of a qualitative study where the purpose was to examine the role of spirituality in developing a positive cultural identity among a multicultural group of 31 adult educators, and then considers what the finding suggest for the further development of culturally relevant teaching practices within adult and higher education settings. In recent years, there has been much discussion about dealing with culture, race, gender, class, sexual orientation in teaching for social change and greater equity in society (Guy, 1999; Hayes & Colin, 1994; Hayes & Flannery, 2000; Johnson-Bailey, 2001). There has also been some discussion of the role of spirituality in adult development and learning (English & Gillen, 2000; Tisdell, 2000), and some limited discussion on the connection between spirituality and teaching for social justice related to cultural issues ( Hart & Holton, 1993; Tisdell, Tolliver, and Villa, 2001; Tolliver & Tisdell, 2002). Most of these discussions have been conceptual in nature, and there has been only limited discussion of the role of spirituality in developing a positive cultural identity from a data-based research perspective. Thus, the purpose of this paper is (1) to discuss the results of a qualitative study where the purpose was to examine the role of spirituality in developing a positive cultural identity among a multicultural group of 31 adult educators; and (2) to discuss the implications the findings of the study have for the further development of culturally relevant teaching practices within adult and higher education settings.Item Effects of Spiritual Care on Well-being of Intensive Care Family Surrogates: A Clinical Trial(Elsevier, 2023) Torke, Alexia M.; Varner-Perez, Shelley E.; Burke, Emily S.; Taylor, Tracy A.; Slaven, James E.; Kozinski, Kathryn L.; Maiko, Saneta M.; Pfeffer, Bruce J.; Banks, Sarah K.; Medicine, School of MedicineContext: Critical illness of a family member is associated with high emotional and spiritual distress and difficult medical decisions. Objectives: To determine if a semistructured spiritual care intervention improves the well-being of family surrogate decision makers in intensive care (ICU) settings. Methods: This study is a randomized, allocation-concealed, parallel group, usual care-controlled, single-blind trial of patient/surrogate dyads in five ICUs in one Midwest, academic medical center. Patients were 18 and older admitted to the ICU and unable to make medical decisions. The intervention involved proactive contact from the chaplain, scheduled, semi-structured visits, weekly follow-up, and bereavement calls. The control group received usual care. The primary endpoint was the surrogate's anxiety (Generalized Anxiety Disorders-7 scale), six to eight weeks after discharge. Results: Of 192 patient/surrogate dyads enrolled, 128 completed outcome assessments. At follow-up, anxiety in the intervention group was lower than control (median score 1 (interquartile range 0,6) vs. 4 (1,9), P = 0.0057). The proportion of patients with a minimal clinically important difference (MCID) in anxiety of 2+ was 65.2% in the intervention group vs. 49.2% in control. The odds of an MCID remained higher in adjusted analysis (Odds Ratio 3.11, 95% confidence interval 1.18, 8.21; P = 0.0218) The adjusted odds of an MCID were higher for spiritual well-being (OR 3.79, CI 1.41,10.17; P = 0.0081). Satisfaction with spiritual care was also higher (adjusted mean 3.5 ± 0.4 vs. 2.9 ± 0.1); P < .0001). Conclusions: Proactive, semistructured spiritual care delivered by chaplains improves well-being for ICU surrogates. Today we will continue this critical discussion.Item Feasibility and acceptability of chaplain decision coaching on Periviable resuscitation decision quality: A pilot study(Elsevier, 2024-02-21) Varner-Perez, Shelley E.; Hoffman, Shelley M.; Coleman-Phox, Kimberly; Bhamidipalli, Sruthi; Monahan, Patrick O.; Kuppermann, Miriam; Tucker Edmonds, Brownsyne; Obstetrics and Gynecology, School of MedicineObjective: To pilot test and assess the feasibility and acceptability of chaplain-led decision coaching alongside the GOALS (Getting Optimal Alignment around Life Support) decision support tool to enhance decision-making in threatened periviable delivery. Methods: Pregnant people admitted for threatened periviable delivery and their 'important other' (IO) were enrolled. Decisional conflict, acceptability, and knowledge were measured before and after the intervention. Chaplains journaled their impressions of training and coaching encounters. Descriptive analysis and conventional content analysis were completed. Results: Eight pregnant people and two IOs participated. Decisional conflict decreased by a mean of 6.7 (SD = 9.4) and knowledge increased by a mean of 1.4 (SD = 1.8). All rated their experience as "good" or "excellent," and the amount of information was "just right." Participants found it "helpful to have someone to talk to" and noted chaplains helped them reach a decision. Chaplains found the intervention a valuable use of their time and skillset. Conclusion: This is the first small-scale pilot study to utilize chaplains as decision coaches. Our results suggest that chaplain coaching with a decision support tool is feasible and well-accepted by parents and chaplains. Innovations: Our findings recognize chaplains as an underutilized, yet practical resource in value-laden clinical decision-making.Item Global Studies of Religiosity and Spirituality: A Systematic Review for Geographic and Topic Scopes(MDPI, 2020-08) Herzog, Patricia SnellThis paper advances the global study of religiosity by conducting a systematic review of the geographic scope, religious traditions, levels of analysis, and topics investigated within contemporary scientific studies of religion, paying particular attention to intersections with generosity. The analysis builds upon a meta-analysis of 30 years of scientific studies of religion that was published ten years ago and engages a similar framework to analyze the most recent ten years of research on religiosity and spirituality. Specifically, this analysis codes for the potential for Western-centrism, Christian-centrism, and congregational-centrism, all while attending to ways to study the potential intersection between religiosity and generosity, especially during the formative youth development life stage. Two data sources inform this analysis: the international data catalog of the Association for Religious Research Archives (ARDA) and the Journal for the Scientific Study of Religion (JSSR). The results indicate that centrism remains, though perhaps to a lesser extent than in the previous decades, with the notable exception of a remaining inequality in the geographic scope. Implications for research are discussed, including practical implications to implementing a better geo-tagging process to more overtly identify the scope of data and make U.S. scope less implicit.Item God Consciousness Enacted: Living, Moving, and Having my Being in Him(Washington State University, 2015) Cannon, Mercedes A.; Morton, Crystal H.Spirituality, historically and contemporarily, has played a crucial role in African American women's navigation of the academy. Drawing on Cozart's (2010) conceptualization of spirituality as God consciousness, this article provides a personal account of an African American graduate student's journey towards her doctoral degree at a predominately white institution. According to Cozart (2010), God consciousness guides one in her interactions within her community and the decisions one makes within those spaces. The authors share how God consciousness provides a lens to perceive, understand, and overcome challenges African American women face in the academy.
