Item A Contextual Approach to the Psychological Study of Identity Concealment: Examining Direct, Interactive, and Indirect Effects of Structural Stigma on Concealment Motivation Across Proximal and Distal Geographic Levels(Sage, 2021) Lattanner, Micah R.; Ford, Jessie; Bo, Na; Tu, Wanzhu; Pachankis, John E.; Dodge, Brian; Hatzenbuehler, Mark L.; Biostatistics, School of Public HealthPsychological theories of identity concealment locate the ultimate source of concealment decisions within the social environment, yet most studies have not explicitly assessed stigmatizing environments beyond the immediate situation. We advanced the identity-concealment literature by objectively measuring structural forms of stigma related to sexual orientation (e.g., social policies) at proximal and distal geographic levels. We linked these measures to a new, population-based data set of 502 gay and bisexual men (residing in 44 states and Washington, DC; 269 counties; and 354 cities) who completed survey items about stigma, including identity-concealment motivation. Among gay men, the association between structural stigma and concealment motivation was (a) observed across three levels (city, county, and state), (b) conditional on one's exposure at another geographic level (participants reported the least motivations to conceal their identity if they resided in both cities and states that were lowest in structural stigma), and (c) mediated by subjective perceptions of greater structural stigma.Item The Impact of Sexual Identity Development on the Sexual Health of Youth Formerly in the Foster Care System(2019-02) Brandon-Friedman, Richard A.; Pierce, Barbara; Fortenberry, J. Dennis; Thigpen, Jeffrey; Wahler, ElizabethYouth in the foster care system receive less sexual and reproductive health education, experience higher levels of negative sexual health outcomes, and engage in more risky sexual behaviors than peers not in the foster care system. Counteracting these concerns requires understanding the processes that contribute to these outcomes. A conceptual model interfacing traditional identity development theories and social constructionist theories of social sexualization was developed that posited sociosexual input factors of sexual education and socialization, sexual abuse history, and adverse childhood experiences affect youths’ sexual identity development, which then impacts youths’ level of sexual health. Hierarchical linear regression determined the level of impact of sexual socialization on sexual health within a sample of youth formerly in the foster care system (n = 219). Whether sexual identity development level mediated the relationship between sexuality-related discussions and sexual health was tested as well as how relationship quality moderates the effects of sexuality-related topic discussions on sexual identity development. Further analysis explored differences between the experiences of youth who identified as sexual minorities and their peers who identified as heterosexual. Results indicated that gender identity, sexual orientation, adverse childhood experiences, sexual abuse history, and sexuality-related discussions with foster parents and with peers all impact sexual health. All four dimensions of sexual identity development significantly contributed to sexual health outcomes. Mediation occurred with two of the four sexual identity development dimensions, whereas no moderation effects were indicated. Youth who identified as sexual minorities and youth who identified as heterosexual had significantly different scores on three of four sexual identity development dimensions and youth who identified as sexual minorities had worse sexual health outcomes. Results indicate the importance of the sexual identity development process on sexual health and that youths’ sexual orientation identity must be considered when designing interventions to improve sexual health outcomes.Item Sexual Health Research With Young Black Men Who Have Sex With Men: Experiences of Benefits and Harms(Springer, 2017-05) Arrington-Sanders, Renata; Morgan, Anthony; Oidtman, Jessica; Dao, Thu (Ann); Moon, Margaret; Fortenberry, J. Dennis; Ott, Mary A.; Pediatrics, School of MedicineYoung Black men who have sex with men (YBMSM) are often underrepresented in sexual health research because of concerns about safety, privacy, and the potential for research harms. Empirical data are needed to understand YBMSM experience of participating in research, benefits and harms (discomfort), to inform policy and regulatory decisions. Using qualitative methods, this article examines 50 YBMSM, aged 15-19 years, experiences of benefits/harms, challenges of participating in sexual health research, and contextual factors impacting research experiences. Participants were asked about benefits and harms experienced in answering questions about sexual orientation, first same-sex attraction, and same-sex sexual experiences after completing an in-depth interview. Interviews were transcribed and coded. Inductive open coding was used to identify themes within and between interviews. Participants were able to describe perceived direct benefits resulting from research interview participation, including awareness of risky sexual behaviors, a safe space to share early coming out stories and same-sex sexual experiences, and a sense of empowerment and comfort with one's sexual orientation. Indirect benefits described by participants included perceptions of helping others and the larger gay community. Few participants described harms (discomfort recalling experiences). Our data suggest that participating in qualitative sexual health research focused on sexual orientation, sexual attraction, and early same-sex sexual experiences may result in minimal harms for YBMSM and multiple benefits, including feeling more comfortable than in a general medical visit.
