Item Developmental Outcomes in Early-Identified Children who are Hard of Hearing at 2 to 3 Years of Age(Wolters Kluwer, 2021) Stika, Carren J.; Eisenberg, Laurie S.; Carter, Alice S.; Johnson, Karen C.; Hammes Ganguly, Dianne M.; Henning, Shirley C.; DesJardin, Jean L.; Otolaryngology -- Head and Neck Surgery, School of MedicineObjectives: This study examined the psychosocial, adaptive behavior, and language outcomes of young children who are hard of hearing (HH) without additional disabilities or neurocognitive impairments. Relations between early developmental outcomes and child and parent demographic variables, and parenting stress and self-efficacy were also explored. Design: Participants were 39 children with early identified, permanent mild to severe hearing loss, between the ages of 2 and 3 years, and a comparison group of 47 children with normal hearing (NH). Developmental outcomes were measured using clinician-administered standardized tests and parent-completed behavior rating instruments specific to language, psychosocial functioning, and adaptive behavior. Mothers completed self-report measures that assess parenting stress and maternal self-efficacy. Results: The children who are HH were similar to the children with NH in terms of their psychosocial functioning and adaptive behavior, with the exception of their socialization skills. As a group, the children who are HH performed significantly worse than their peers with NH on all measures of language ability. Among the children who are HH, maternal self-efficacy showed a strong positive correlation with adaptive behavior outcomes; however, it failed to contribute unique variance above that explained by language ability and gender. Maternal self-efficacy was also significantly correlated with better psychosocial outcomes, but only parenting stress proved to be a significant predictor of child behavioral problems once other variables considered were in the model. Conclusions: Early-identified young children who are HH can demonstrate age-appropriate development in multiple domains, including language, psychosocial, and adaptive behavior. However, mild to severe hearing loss places young children with no additional disabilities or neurocognitive impairments at risk for language delays. Although the children who are HH demonstrated no more emotional or behavioral problems than their same-age peers with NH, results suggest that language delays increase their vulnerability for delays in various aspects of social competence.Item Relationship Between Extreme Pain Phenotypes and Psychosocial Outcomes in Persons With Chronic Pain Following Traumatic Brain Injury: A NIDILRR and VA TBI Model Systems Collaborative Project(Wolters Kluwer, 2024) Ketchum, Jessica M.; Hoffman, Jeanne M.; Agtarap, Stephanie; Hammond, Flora M.; Martin, Aaron M.; Walker, William C.; Zafonte, Ross; Harrison-Felix, Cynthia; Nakase-Richardson, Risa; Physical Medicine and Rehabilitation, School of MedicineObjective: To examine the relationship between extreme pain phenotypes (interference and improvement) and psychosocial outcomes among those with chronic pain after traumatic brain injury (TBI). Setting: Community. Participants: In total, 1762 TBI Model Systems (TBIMS) participants 1 to 30 years postinjury reporting chronic pain. Design: Multisite, cross-sectional, observational cohort study. Primary measures: Life satisfaction, posttraumatic stress, depression and anxiety symptoms, sleep and participation, the Brief Pain Inventory (BPI) interference scale, and the Patient's Global Impression of Change (PGIC). Results: Persons in the extreme high interference phenotype (vs extreme low interference phenotype) and/or extreme no change phenotype (vs extreme improvement phenotype) had poorer psychosocial outcomes, with extreme pain interference phenotypes having a larger effect on outcomes than extreme perceived improvement phenotypes. After controlling for covariates, large effect sizes (ES) related to pain interference were observed for posttraumatic stress symptoms (ES = -1.14), sleep quality (ES = -1.10), depression (ES = -1.08), anxiety (ES = -0.82), and life satisfaction (ES = 0.76); effect sizes for participation outcomes, although significant, were relatively small (ES = 0.21-0.36). Effect sizes related to perceived improvement were small for life satisfaction (ES = 0.20) and participation (ES = 0.16-0.21) outcomes. Pain intensity was identified as a meaningful confounding factor of the relationships between extreme phenotypes and posttraumatic stress, depression, anxiety, and sleep quality. Conclusions: Examination of extreme phenotypes provides important insights into the experience of individuals living with chronic pain and TBI. Results suggest that the relationships among a variety of characteristics of the person, their experience with pain, and treatment of pain are complex. Further research is needed to better understand these complex relationships and how differences in pain interference and perceived improvement from treatment can assist in assessment and treatment of chronic pain after TBI.Item Vocational and Psychosocial Outcomes of Work Re-Integration Programs for Individuals with Severe Mental Illness: A Rapid Systematic Review(2020-04) Bender, Brooke; Brown, Hannah; Davis, Madyson; Lester, Lauren; Smith Jr., Carlton; Chase, Tony; Department of Occupational Therapy, School of Health and Human SciencesThere are an estimated 11.2 million adults in the United States with a serious mental illness and this population is 6-7 times more likely to be unemployed. A systematic review of the literature related to work re-integration programs for individuals with severe mental illness was conducted to determine the role of occupational therapy in this area of practice. This included a comprehensive review of 25 studies that addressed many of the interventions commonly used in vocational and supported employment programs for individuals with severe mental illness and related mental health disorders. Findings reveal that various types of work re-integration programs result in competitive work attainment, increased job tenure, improved quality of life, and improved psychosocial factors. Occupational therapy could facilitate interventions related to work re-integration that improve vocational and non-vocational outcomes for this population. The literature over work re-integration programs is limited and more research is needed in order to understand the correlation between those with SMI and employment outcomes.
