Item Active Gaming and Energy Expenditure in Healthy Adults(Office of the Vice Chancellor for Research, 2016-04-08) Carey, Christopher; Naugle, Keith; Ohlman, Tom; Mikesky, Alan; Naugle, KellyThe rise in sedentary behavior in US society, along with the associated health risks, makes it necessary to find effective solutions to increase physical activity at all levels. Studies on active gaming have shown promising results in the use of active gaming as a viable exercise tool that combines physical activity with entertainment. However, the research is still mixed on whether active games can elicit similar responses as traditional cardiovascular exercise, such as jogging on a treadmill. This study examined whether participants playing active games could reach a moderate intensity level as defined by the American College of Sports Medicine as 3.0 METs while playing with and without specific instructions designed to maximize physical activity. Twenty young adult participants completed one training session and four experimental sessions. During each session, participants played two 15-minute periods of either Kinect tennis, Kinect boxing, Wii tennis, and Wii boxing. In period one, participants played at a self-selected intensity. During period two, participants were given specific instructions on how to play which were designed to maximize movement during play and down time. During game play, participants wore a portable gas (VO2/VCO2) analyzer to measure energy expenditure. Metabolic equivalents (METS) were analyzed with a repeated measures ANOVA. During period 1, Kinect boxing was able to elicit the highest METs, 3.097±0.3, from the participants. METS during period 2 was significantly greater than during period 1 across all games (p<.001). Participants were able to reach higher than 3.0 METs while playing each of the games during period 2. Regardless of the period, Kinect boxing elicited greater METS than Wii boxing and Wii Tennis (P<.001). This shows evidence that active gaming not only can elicit a moderate intensity level of physical activity, but that specialized instructions can enhance the effects of the active games.Item EFFECTS OF KAATSU TRAINING ON UPPER EXTREMITY SIZE AND STRENGTH(Office of the Vice Chancellor for Research, 2012-04-13) Weatherholt, Alyssa M.; Greer, Stephanie A.; Ruark, Dana L.; Grider, Blake J.; Mock, Marla J.; Beekley, Matthew D.; Mikesky, AlanConventional resistance training involves lifting heavy loads (~70% max-imal strength), which can be poorly tolerated or contraindicated in many clinical populations. KAATSU training is a novel training mode from Japan that combines muscle blood flow restriction with low load lifting (~20% max-imal strength). The purpose of this study was to investigate the effects of a KAATSU training program on upper extremity size and strength. Forty healthy subjects ages 18 to 30 were divided into exercise (EX) or control (CON) groups. Subjects reported to the laboratory three times per week for eight weeks. The EX group performed 3 sets of 15 repetitions of unilateral bicep curls and triceps extensions lifting loads equivalent to 20% of their predetermined maximal strength while wearing a pneumatic cuff to restrict blood flow on one arm (CUFF) and nothing on the other (NCUFF). The CON group did not perform any exercises but wore the cuff on one arm for a time comparable to the EX group. CUFF and NCUFF arms were randomly as-signed. Strength, girth, skin folds and tomography scans were taken pre-, during, and post-eight weeks. In the EX group, bicep curl (17.4% +4.1% and 18.7% +4.9%) and triceps extension (15.8% +3.4% and 10.7% +2.7%) strength increased significantly over the 8-week period for both the CUFF and NCUFF arms, respectively. No significant differences in strength occurred between the CUFF and NCUFF arms within the group. No strength changes were noted in the CON group for the CUFF and NCUFF arms. Arm girth and muscle cross-sectional area (mCSA) increased significantly in the EX subjects compared to the CON subjects, however no significant differ-ences were found when within group comparisons were made between the CUFF and NCUFF arms. This study indicates that KAATSU training can in-crease muscle strength and mCSA.Item Effects of physical training on proprioception in older women(2003) Thompson, KR; Mikesky, Alan; Bahamonde, Rafael E.; Burr, David B.Older adulthood is accompanied by declines in muscular strength, coordination, function, and increased risk of falling. Resistance training increases muscular strength in this population but its effect on proprioception is unknown. To evaluate the effect of resistance training on proprioception, community dwelling older women completed a three-month exercise study. A resistance training (RT) group (N=19) underwent supervised weight training three times per week while a non-strength trained control (NSTC) group (N=19) performed range-of-motion activities that mimicked the movements of the RT group without the benefit of muscle loading. Subjects were evaluated at baseline, 6, and 12 weeks for strength and proprioception. Muscular strength was assessed by measuring the subject’s one repetition maximum performance on four different exercises. Static proprioception was measured by the subject’s ability to reproduce a target knee joint angle while dynamic proprioception was measured by the subject’s ability to detect passive knee motion. The RT group made significant strength improvements compared to the NSTC group. Proprioception significantly improved in both groups by 6 weeks. Our findings suggest that improvements in proprioception can be obtained via regular activity that is independent of heavy muscle loading.Item KAATSU TRAINING: PERCEPTIONS AND COMPLIANCE TO AN UPPER ARM EXERCISE PROGRAM(Office of the Vice Chancellor for Research, 2012-04-13) Mock, Marla J.; Weatherholt, Alyssa M.; Greer, Stephanie A.; Ruark, Dana L.; Grider, Blake J.; Urtel, Mark; Beekley, Matthew D.; Mikesky, AlanKAATSU training is a novel Japanese training mode involving low load (~20% maximal strength) weightlifting combined with blood flow restriction to the muscles. Little is known about the sensations experienced during KAATSU training and whether clients will tolerate it. The purpose of this study was to assess perceived sensations, exertion rates, and compliance to an upper arm KAATSU training program. Forty healthy subjects ages 18 to 30 were divided into exercise (EX) or control (CON) groups. Subjects report-ed to the laboratory three times per week for eight weeks. The EX group performed 3 sets of 15 repetitions of unilateral bicep curls and triceps exten-sions lifting loads equivalent to 20% of their predetermined maximal strength while wearing a pneumatic cuff on one arm (CUFF) and nothing on the other (NCUFF). The CON group did not perform any exercises but wore the cuff on one arm for a time comparable to the EX group. CUFF and NCUFF arms were randomly assigned. Sensations (burning, aching, pressure, pins & needles) and perceived exertion were assessed using visual analog scales that included emotions and verbal cues. Compliance (percentage of subjects completing the pre- and post-testing) and adherence (percentage of ses-sions completed) were tracked to provide an indication of training tolerance. Compliance was 85.4% and 97% for the EX and CON groups, respectively. EX subjects completed 85.4% of their workouts while controls attended 90.4% of their sessions. The prominent sensation reported in the CUFF arm was pressure (moderate; 3.2 +0.6) followed by aching (weak; 1.7 +0.4). Ratings of perceived exertion were higher for the CUFF (3.2 +1.0, 5.1 +1.8, and 7.0 +2.5) versus NCUFF (1.5 +0.3, 2.4 +0.3, and 3.3 +0.4) arm for sets 1, 2, and 3, respectively. KAATSU training is well tolerated by those performing it and a viable alternative to conventional resistance exercise.Item Predictors of Health-related Quality of Life at One Month after Head and Neck Cancer Surgery(Office of the Vice Chancellor for Research, 2011-04-08) Wang, Hsiao-Lan; Mikesky, Alan; Keck, Juanita F.; Bunnell, Karen; Buelow, Janice; Rawl, Susan M.Significance: Head and neck cancer (HNC) survivors frequently experience treatment-related complications that may cause decrements in health-related quality of life (HRQOL). Before interventions can be designed to enhance HRQOL in the early postoperative period, descriptive research is needed to determine predictors of HRQOL in this understudied population. Problem and Purpose: The proposed study identified predictors of global HRQOL, and physical, functional, emotional, and social well-being at one month after HNC surgery. Variables examined as potential predictors included shoulder pain, shoulder function, and functional impairments (disfigurement, and eating, speech, and breathing impairments). Theoretical Framework: The University of California, San Francisco School of Nursing Symptom Management Model was modified and used to guide the study. Methods and Analysis: In this exploratory, cross-sectional study, we examined a convenience sample of 29 patients who had undergone HNC surgery with curative intent one month previously. Global HRQOL was measured using the Functional Assessment of Chronic Illness Therapy (FACIT) General Scale, including four well-being subscales. Shoulder pain intensity was measured using the Brief Pain Inventory, shoulder pain distress was measured using a 0-10 numerical rating scale, and functional impairment was measured using the FACIT Head and Neck Subscale. Shoulder function was assessed using a goniometer. Pearson correlations were initially applied to determine correlates (p <0.20) that should be entered in subsequent stepwise regression models. Findings and Implications: The only significant predictor of global HRQOL was eating impairment (B = -0.20, p = 0.02). Predictors of physical well-being were shoulder pain distress (B = -0.10, p = 0.02) and eating impairment (B = 0.27, p = 0.03). Predictors of functional well-being were speech impairment (B = -0.43, p < 0.01) and disfigurement (B = - 0.20, p = 0.02). No significant predictors were found for emotional and social well-being. The findings suggested that patients’ physical and functional well-being can be influenced by eating impairment, shoulder pain distress, speech impairment, or disfigurement at one month after HNC surgery. Nurses need to monitor nutrition intake, provide proper pain management, and collaborate with speech and physical therapists to promote early rehabilitation. A longitudinal study with a larger sample size is warranted to describe needs for multidisciplinary care to improve HQOL after HNC surgery.Item Proprioception, gait kinematics, and rate of loading during walking: Are they related?(2005) Riskowski, JL; Mikesky, Alan; Bahamonde, Rafael E.; Alvey III, TV; Burr, David B.The cyclic nature of walking can lead to repetitive stress and associated complications due to the rate of loading (ROL) experienced by the body at the initial contact of the foot with the ground. An individual’s gait kinematics at initial contact have been suggested to give rise to the ROL, and a repetitive, high ROL may lead to several disorders, including osteoarthritis. Additionally, proprioception, the feedback signaling of limb position and movement, may play a role in how the foot strikes the ground and thus, the ROL. Our goal was to explore the relationship between proprioception, gait kinematics and ROL. Thirty-eight women were recruited for gait analysis, and the gait characteristics 50ms prior to and at initial contact were examined. Two proprioception tests, joint angle reproduction and threshold to detect passive motion were used to examine the subject’s proprioceptive acuity. Our results indicate that individuals with a larger knee angle (i.e., greater extension) 50ms prior to initial contact (IC) experience a higher ROL during gait and have poorer proprioceptive scores. However, it remains unclear whether poor proprioception causes a high ROL or if a high ROL damages the mechanoreceptors involved in proprioception, but the apparent relationship is significant and warrants further investigation.Item Shoulder Pain, Functional Status, and Health-Related Quality of Life after Head and Neck Cancer Surgery(2013-12) Wang, Hsiao-Lan; Keck, Juanita F.; Weaver, Michael T.; Mikesky, Alan; Bunnell, Karen; Buelow, Janice M; Rawl, Susan M.Head and neck cancer (HNC) patients experience treatment-related complications that may interfere with health-related quality of life (HRQOL). The purpose of this study was to describe the symptom experience (shoulder pain) and functional status factors that are related to global and domain-specific HRQOL at one month after HNC surgery. In this exploratory study, we examined 29 patients. The outcome variables included global HRQOL as well as physical, functional, emotional, and social well-being. Symptom experience and functional status factors were the independent variables. In the symptom experience variables, shoulder pain distress was negatively associated with physical well-being (R2 = 0.24). Among the functional status variables, eating impairment was negatively related to global HRQOL (R2 = 0.18) and physical well-being (R2 = 0.21). Speaking impairment and impaired body image explained a large amount of the variance in functional well-being (R2 = 0.45). This study provided initial results regarding symptom experience and functional status factors related to poor HRQOL in the early postoperative period for HNC patients.
