Students will review sexual health and risks to sexual health. SESSION OBJECTIVES: Describe the definitions of sexual health promoted by the World Health Organization and Centers for Disease Control; explain the four phases of the sexual response cycle (excitement, plateau, orgasm and resolution) in women and men; compare and contrast sexual dysfunction in men and women; list the various sexual practices engaged in by men and women; and explain the health risks associated with various sexual practices, and the interventions recommended to reduce this risk.Item Paracrine IL-2 Is Required for Optimal Type 2 Effector Cytokine Production(American Association of Immunologists, 2017-06-01) Olson, Matthew R.; Ulrich, Benjamin J.; Hummel, Sarah A.; Khan, Ibrahim; Meuris, Brice; Cherukur, Yesesri; Dent, Alexander L.; Janga, Sarath Chandra; Kaplan, Mark H.; Pediatrics, School of MedicineIL-2 is a pleiotropic cytokine that promotes the differentiation of Th cell subsets, including Th1, Th2, and Th9 cells, but it impairs the development of Th17 and T follicular helper cells. Although IL-2 is produced by all polarized Th subsets to some level, how it impacts cytokine production when effector T cells are restimulated is unknown. We show in this article that Golgi transport inhibitors (GTIs) blocked IL-9 production. Mechanistically, GTIs blocked secretion of IL-2 that normally feeds back in a paracrine manner to promote STAT5 activation and IL-9 production. IL-2 feedback had no effect on Th1- or Th17-signature cytokine production, but it promoted Th2- and Th9-associated cytokine expression. These data suggest that the use of GTIs results in an underestimation of the presence of type 2 cytokine-secreting cells and highlight IL-2 as a critical component in optimal cytokine production by Th2 and Th9 cells in vitro and in vivo.Item Pathological Complete Response of Metastatic Clear Cell Renal Carcinoma with Pembrolizumab and Axitinib: A Case Report and Review of Literature(Karger, 2023-02-02) Beirat, Amir F.; Menakuru, Sasmith R.; Khan, Ibrahim; Siddiqui, Salahuddin; Medicine, School of MedicineThe role of cytoreductive nephrectomy has become unclear since the introduction of immunotherapy which is now the backbone of the treatment for metastatic renal cell carcinoma. Different combinations are used based on the prognosis. Achieving a complete response would be ideal and includes radiographic disappearance of lesions. However, there have been a few reported cases of pathological complete response with persistent radiographic evidence of cancer. The authors report a case of pathological complete response despite persistent radiographic evidence of residual disease in a patient with metastatic renal cell carcinoma treated with pembrolizumab and axitinib. The patient subsequently underwent cytoreductive nephrectomy after the 13th dose of pembrolizumab. The resected mass consisted of scar tissue with no viable tumor cells seen on pathology but only scar tissue. This case reveals that persistent radiographic evidence of the tumor may be explained by scar tissue, challenging the role of cytoreductive nephrectomy in the era of immunotherapy.
