Item Cardiovascular-Related Outcomes in U.S. Adults Exposed to Lead(MDPI, 2018-04) Obeng-Gyasi, Emmanuel; Armijos, Rodrigo X.; Weigel, M. Margaret; Filippelli, Gabriel M.; Sayegh, M. Aaron; Earth Sciences, School of ScienceCardiovascular-related clinical markers were evaluated in this cross-sectional study of United States adults (aged ≥ 20) exposed to lead via the National Health and Nutrition Examination Survey 2007–2008 and the 2009–2010 datasets. In four quartiles of exposure—0–2 μg/dL, 2–5 μg/dL, 5–10 μg/dL, and 10 μg/dL and over, clinical and anthropometric markers were evaluated—to examine how the markers manifested in the quartiles. Associations were determined via linear regression. Finally, clinical makers, and how they manifested between exposed and less-exposed occupations, were explored in addition to how duration of exposure altered these clinical markers. In regression analysis, Diastolic Blood Pressure (DBP) and high-density lipoprotein (HDL) cholesterol, were significantly associated with blood lead level (BLL). In the occupational analysis, Systolic Blood Pressure (SBP), DBP, C-reactive protein (CRP), triglycerides, low-density lipoprotein (LDL) cholesterol, high-density lipoprotein (HDL) cholesterol, showed differences between populations in the exposed and less-exposed occupations. Regarding Agriculture, Forestry & Fishing, the duration of exposure altered SBP, CRP, and LDL cholesterol. With mining, the duration of exposure altered SBP, DBP, triglycerides, and HDL cholesterol, whereas in construction, the duration in occupation altered SBP, triglycerides, and CRP. In conclusion, lead exposure has a profound effect on the cardiovascular system, with potentially adverse outcomes existing at all exposure levels.Item Chronic Occupational Exposure to Traffic Pollution Is Associated with Increased Carotid Intima-Media Thickness in Healthy Urban Traffic Control Police(MDPI, 2023-09-01) Balogun, Abdulrazak O.; Weigel, M. Margaret; Estévez, Edmundo; Armijos, Rodrigo X.; Community and Global Health, School of Public HealthUrban traffic officers in many low- and middle-income countries are exposed to high levels of traffic-related air pollutants (TRAP) while working vehicle control on heavily congested streets. The impact of chronic TRAP exposure on the cardiovascular health, including the carotid intima-media thickness (CIMT), of this outdoor occupational group remains unclear. This cross-sectional study compared the average mean and maximum CIMT measurements of two groups of relatively young, healthy traffic police (32 ± 7 years; 77% male) in Quito, Ecuador, who were without clinical evidence of serious cardiovascular or other disease. Previously published background data on PM10 (a TRAP surrogate) indicated that street levels of the pollutant were several orders of magnitude higher at the street intersections worked by traffic police compared to those working only in an office. Accordingly, officers permanently assigned to daily traffic control duties requiring them to stand 0–3 m from heavily trafficked street intersections were assigned to the high exposure group (n = 61). The control group (n = 54) consisted of officers from the same organization who were permanently assigned to office duties inside an administration building. Mean and maximum CIMT were measured with ultrasound. General linear models were used to compare the CIMT measurements of the high exposure and control groups, adjusting for covariates. The adjusted average mean and maximum CIMT measures of the high exposure group were increased by 11.5% and 10.3%, respectively, compared to the control group (p = 0.0001). These findings suggest that chronic occupational exposure to TRAP is associated with increased CIMT in traffic police. This is important since even small increases in arterial thickening over time may promote earlier progression to clinical disease and increased premature mortality risk.Item Hepatobiliary-Related Outcomes in US Adults Exposed to Lead(MDPI, 2018-03-31) Obeng-Gyasi, Emmanuel; Armijos, Rodrigo X.; Weigel, M. Aaron; Earth Sciences, School of ScienceThe purpose of this cross-sectional study was to investigate hepatobiliary-related clinical markers in Unites States adults (aged ≥ 20) exposed to lead using the National Health and Nutrition Examination Survey (NHANES) 2007–2008 and 2009–2010 datasets. Clinical markers and occupation were evaluated in 4 quartiles of exposure—0–2 μg/dL, 2–5 μg/dL, 5–10 μg/dL, and 10 μg/dL and over—to examine how the markers and various occupations manifested in the quartiles. Linear regression determined associations, and binary logistic regression predicted the likelihood of elevated clinical makers using binary degrees of exposure set at (2 μg/dL, 5 μg/dL, and 10 μg/dL). Clinical makers, and how they manifested between exposed and less-exposed occupations, were explored in addition to how duration of exposure altered these clinical markers. In regression analysis, Gamma-Glutamyl Transferase (GGT), total bilirubin, and Alkaline Phosphatase (ALP) were positively and significantly associated with Blood lead level (BLL). Using binary logistic regression models, at the binary 2 μg/dL level ALP, and GGT were more likely to be elevated in those exposed. At 5 μg/dL level, it was ALP and GGT that were more likely to be elevated in those exposed whereas at 10 μg/dL level, it was GGT that were more likely to be elevated in those exposed. In the occupational analysis, Aspartate Aminotransferase (AST), Alanine Aminotransferase (ALT), GGT, and ALP showed differences between populations in the exposed and less-exposed occupations. Regarding Agriculture, Forestry and Fishing, duration of exposure altered AST, ALP, and total bilirubin significantly (p < 0.05) while ALT and GGT were altered moderately significantly (p < 0.10). With mining, duration of exposure altered AST and GGT moderately significantly, whereas in construction duration in occupation altered AST, and GGT significantly, and total bilirubin moderately significantly. The study findings are evidence of occupational exposure to lead playing a significant role in initiating and promoting adverse hepatobiliary clinical outcomes in United States adults.Item Household food and water insecurity and its association with diarrhoea, respiratory illness, and stunting in Ecuadorian children under 5 years(Wiley, 2024) Chakraborty, Rishika; Armijos, Rodrigo X.; Beidelman, Erika T.; Rosenberg, Molly; Weigel, M. Margaret; Center for Global Health Equity, School of MedicineHousehold food and water insecurity has been previously associated with adverse health consequences in children. However, these relationships are understudied in middle-income Latin American populations such as in Ecuador, where a high prevalence of food and water insecurity has been reported. Using cross-sectional data from 2018 Ecuadorian National Health and Nutrition Survey, we examined the association of household food insecurity (HFI), household water insecurity (HWI), and concurrent HFI-HWI with diarrhoea, respiratory illness (RI), and stunting in 20,510 children aged ≤59 months. HFI was measured using the Food Insecurity Experience Scale. HWI was defined when households responded negatively to one or more of four drinking water indicators. Maternal caregivers reported on child diarrhoea and RI episodes during the previous 2 weeks. Measured length or height was used to assess stunting. We constructed log-binomial regression models to estimate the associations of HFI, HWI, and concurrent HFI-HWI with child outcomes. Moderate-severe HFI was associated with a higher prevalence of diarrhoea (PR = 1.39; 95% CI: 1.18, 1.63) and RI (PR = 1.34; 95% CI: 1.22, 1.47), HWI with a higher prevalence of RI (PR = 1.13; 95% CI: 1.04, 1.22), and concurrent HFI-HWI with a higher prevalence of diarrhoea (PR = 1.30; 95% CI: 1.05, 1.62) and RI (PR = 1.45; 95% CI: 1.29, 1.62). Stunting was not associated with HFI, HWI nor concurrent HFI-HWI. These findings suggest that HFI and HWI can independently and jointly act to negatively affect children's health. Policies and interventions aimed at alleviating both food and water insecurity are needed to bring sustained health improvements in Ecuadorian children.
